Under the general direction of the Manager or designate, provides an accessible, client-centered and integrated approach to the processing of requests for Home Care services; performs initial consultation, preliminary assessment and identification of needs for potential clients; takes and screens referrals from various sources; determines eligibility of clients, initial interventions and the response time needed. This position is a new position as such requires the successful candidate to identify improvements needed in existing process; and to use creativity and resourcefulness to help improve response time, and ensure appropriate and effective client care in the community. Integrating new and improved intake procedures into the present environment will be a key component of the position.
Job QualificationsRequired Qualifications
- Bachelor Degree/Diploma in Nursing or Bachelor Degree/Diploma in Psychiatric Nursing
- Registered or eligible for registration with the College of Registered Nurses of Saskatchewan (CRNS) or Registered Psychiatric Nurses Association of Saskatchewan (RPNAS)
- Valid Class 5 driver's license
Experience
Two (2) years recent experience in Home Care. Knowledge & Skills: - Two years recent, related experience in Home Care; - Knowledge of acute, chronic, rehabilitation and palliative health conditions; - Working knowledge of Home Care Case Management; - Basic computer skills and a willingness to learn Procura and RAI-HC - Knowledge of community resources and health related support services; - Knowledge of Home Care policies and procedures as a component of a community care system within the broade
