Job Description

Reporting to the PHC Regional Director (or a designate such as Community Health Services Manager), Primary Health Care Teams and in collaboration with physicians and health care provider team members, the Registered Nurse (Nurse Practitioner) will provide Primary Health Care (PHC) services to assigned populations which may include: urban populations, visiting services to rural communities, and/or priority populations (e.g. newcomers, teens, residents in long term care facilities) as determined by community and program needs. Depending on community and team requirements, the position may include first on-call support. As the PHC Team evolves, and based on needs of the population, the position may be required to provide new practices such as nurse led extended hour clinic services and/or first on-call support. Primary Health Care services include: health assessment, health promotion and education, disease and injury prevention and assessment, treatment interventions; collaboration with, and referral to, other health care providers; participation in Lean, patient safety, community development, clinical practice redesign, quality improvement, and evaluation.

Job Qualifications

Required Qualifications

  • Advanced Cardiac Life Support (ACLS)
  • Basic Life Support (BLS)
  • Registered Nurse - Nurse Practitioner Masters Program
  • Registered or eligible for registration with the College of Registered Nurses of Saskatchewan (CRNS)
  • Valid Class 5 driver's license

Experience
Minimum of three (3) years experience working as a Registered Nurse, with experience as RN (NP) in a Primary Health Care setting preferred.
Knowledge, Skills and Abilities
  • Ability to competently use computers and software applications as required by the position
  • Ability to develop and maintain good interpersonal relationships and communication with other health care professionals
  • Ability to make decisions, which must include coordination of staffing, and/or patient care, and/or coordination of equipment
  • Ability to work independently and as a member of a multi-disciplinary team
  • Communication skills to provide/obtain verbal and/or written opinions/advice
  • Computer skills in such programs as Microsoft Office Suite (Word, Excel, PowerPoint, etc)
  • Interpersonal skills
  • Knowledge of community development
  • Leadership skills
  • Working knowledge in health promotion

Will provide visiting services to sites within the service area. The service area is subject to change. The Saskatchewan Health Authority (SHA) requires a criminal record check (CRC) from every potential SHA team member. The CRC must be dated within the past six (6) months and be satisfactory to the SHA. A vulnerable sector check (VSC) is required for individuals providing patient/resident/client care to vulnerable persons.
