Job Description:
The Nurse Navigator is accountable for the system navigation and coordination of breast cancer patients. The Nurse Navigator is an expert resource and support for patients and is responsible for the coordination, development, implementation, delivery and evaluation of the nursing component of the Breast Health Program. The Nurse Navigator will work in collaboration with other health care providers including community partners and agencies to promote care based on best practice and will help develop and foster these relationships.
The Nurse Navigator reports to the Manager of Surgical Pathways and acts in accordance with established SHA policies and the College of Registered Nurses of Saskatchewan (CRNS) Standards of Practice.
Human Resources Exemption: No
Experience:
- Minimum of five (5) years of recent breast health care experience is required along with solid knowledge of breast cancer care and breast disease.
- Experience related to leadership, patient education, program development and evaluation is an asset
Education:
- Bachelor of Science in Nursing (BScN)
Other Education and Training
- Nursing diploma is acceptable
Knowledge, Skills, & Abilities:
- Demonstrates superior ability to communicate effectively to provide information, interpretation of diagnostic and treatment options, diagnostic follow-up coordination and emotional support to clients and their families.
- Requires highly developed assessment skills, demonstrates ability to complete initial and ongoing client assessments (clinical and diagnostic reasoning) and provide best client care through appropriate/prescribed technical, therapeutic, safety interventions.
- Independent judgement required to evaluate pertinent client data from multiple sources, plans and develops creative solutions to diverse and complex problems.
- Practices as an expert in the area of Breast Cancer Care and maintains that expertise through on-going professional development. Experience, Training and education reflect a strong interest in Breast Cancer and Oncology care. Demonstrated ability to integrate relevant evidence based best practice as standard performance within the program.
- Sensitive to diverse cultures and preferences; acts as a client advocate and consistently promotes cooperation and collaboration with others.
- Ability to transmit information intended to instruct clients and others about topics essential to client’s plan of care and general health and well-being.
- Demonstrates competency in organizational leadership, management and project development skills. Ability to manage time and various resources efficiently and effectively. Managing competing priorities by demonstrating the ability to multitask through client care responsibilities, simultaneous program development projects and overall program evaluation.
- Flexible in the implementation of new technology within the program and demonstrates an ability to utilize word processing, spreadsheet and database programs.
