Job Description: The Chronic Condition Nurse(CCN) integrates knowledge, skills and the principles of teaching and learning to support healthy lifestyle choices and chronic disease prevention, education and management services for clients, families, and the general public and health professionals.  The CCN uses a consultative approach, accepted standards and guidelines and current research findings to provide support to clients and health care professionals as appropriate.

Education: 

  • Degree in education/adult learning from recognized institution
  • Bachelor of Science in Nursing (BScN)


Competencies: 

  • Computer skills
  • Communication skills

 Experience: 

  • 3 yrs exp chronic disease/complex illness edu. or chronic disease mgnt required
  • Experience as a diabetes nurse educator with renal and cardiovascular disease


Other Education & Training: 

Nephrology Certification is desired
 
Knowledge & Abilities:  
  • Competent critical thinking/judgement/autonomous decision making capabilities
  • Knowledge of motivational interviewing
  • Organize/prioritize work on multiple projects with competing deadlines
  • Knowledge and application of quality improvement principles, tools, and outcomes
  • Knowledge in LEAN management system
  • Systems thinking approach to health care delivery
  • Proven conflict resolution skills

Advanced knowledge of diabetes practices and principles.
