Job Description: The Patient Educator responsibilities include: coordinate and participate in education for Chronic Kidney Disease patients requiring peritoneal dialysis and home hemodialysis. This will include liaison with families, physicians, dieticians, staff nurses and community agencies. Provide individual out-patient education for patients and families in a clinic setting. Promote continuing staff nurse involvement in patient education through effective communication, development of resource materials and provision of in-service education. Coordinate and participate in education programs for designated nursing staff that will be providing peritoneal dialysis and home hemodialysis for patients at St. Pauls Hospital and other agencies. Serve as primary clinical resource for nursing, medical and other professional/support staff. Participate in departmental activities such as program development, planning, quality improvement, and evaluation. Participate in research and evaluation projects.
Human Resources Exemption: No
Experience
- Three (3) years nursing experience with patients/families with chronic disease
- Experience with Peritoneal Dialysis and Hemodialysis
Education
- Bachelor of Science in Nursing (BScN)
Other Education and Training
- CNA certificate in clinical practice area is an asset
- Nursing diploma is acceptable
- Master of Nursing, Adult or Continuing Education is an asset
- Basic Life Support Certificate (BLS)
Competencies
- Advanced - Assessment skills
- Basic - Communication skills to transfer knowledge/skills
Knowledge and Abilities
- Ability to competently use computers and software applications
- Ability to work independently
