Job Description: Reporting to the Manager, Continuing Care Program Services, Rehab Therapies/Outpatient Clinic, the Community Outpatient Rehabilitation Nurse is responsible for ensuring the highest standard of care and service is provided to the client's and families served by Adult Rehab Services. The Community Outpatient Rehabilitation Nurse is a member of the transdisciplinary Adult Rehab Services Team. The incumbent will be required to use their acquired knowledge, clinical expertise and strong interpersonal skills to provide assessments, treatment and alternative service referral recommendations for individuals with a variety of rehabilitation needs and their families. The incumbent will provide expert advice to the Adult Rehab Services staff on the management of individual and family needs from a medical and rehabilitation nursing perspective. He or she will assist in the teaching, training and supervision of nursing students as required. In addition, within the community-based Adult Rehab Services Team, the incumbent will provide consultation to other practitioners and service providers within our service delivery area as required.

- Bachelor of Science in Nursing (BScN)

- Expert knowledge of the causes and effects of disabilities which require rehabilitation.
- Expert knowledge of best practice or empirically based models of assessment and treatment of rehabilitation diagnoses.
- Committed to the promotion and achievement of successful community integration and improved quality of life.
- Knowledge of the aboriginal/multicultural populations.
- Extensive knowledge of community resources for children, youth and families.
- Knowledge of the legislation which governs our practices. Health Information Protection Act (HIPA), Mental Health Act, Substitute Health Care Decision Maker's Act, Powers of Attorney Act, Public Trustee's Act, Adult Guardianship and Co-Decision Maker's Act.
- Leadership abilities and professional maturity to develop and maintain consulting relationships within a transdisciplinary team, interagency programs and across the community of human services.
- Ability to work self-directed and collaboratively as a member of a transdisciplinary team and independently to manage complex and multiple demands.
- Demonstrated skill in mentoring, training and clinically supervising nursing students and related disciplines.
- Ability to design and conduct in-service training for nurses and other professionals.
- Experience in clinical research, program development and evaluation.
- Highly developed written and oral communication skills.
- Efficient basic computer operation skills.
- Effective time management skills and organizational skills. Must be flexible and able to work in non-traditional roles and work places. Ability and willingness to travel if needed within our service delivery area to facilitate patient care.
Experience:
- Current knowledge and three (3) years recent nursing experience in community-based, inpatient, or clinic-based adult rehabilitation.
