Job Description: Ensures provision of safe, comprehensive care, meeting client and staff needs through coordination and communication. Provides assistance with system navigation and the coordinator of patient flow.

Experience
- 5 years nursing experience in primary care, public/community health nursing
- 5+ years experience in healthcare organization and community health service
Education
- Current registration with CRNS and current BCLS certitication.
- Course credits in community health and public health nursing are required
- Bachelor's Degree
Licenses
- Valid Class 5 driver's license
- Registered Nurse (RN)
Other Education and Training
- Experience in program planning and evaluation an asset

- Knowledge in community health nursing theory and principles
- Knowledge in population health promotion approaches and community development
- Policies, Procedures, Regulations and Legislation pertaining to area of practice
- Demonstrated understanding of community development
- Highly developed written and oral communication skills
- ability and willingness to travel within service delivery area
- Ability to work independently and as a member of a team
- Knowledge of communicable disease control practices and procedures
- Demonstrates and is recognized for strategic leadership
- Ability to manage programs within budgetary parameters
- Knowledge of agents, patterns, control issues & the epidemiology of disease
- Excellent management, leadership, organizational and decision making skills
- Adaptability for competing/changing patient priorities, situations, environment
- Knowledge in population and public health concepts
- Knowledge in LEAN management system
- Ability to be flexible and adaptable to changing workload
- Knowledge of epidemiological principles, statistics and infectious disease
