Job Description: The Clinical Coordinator is a skilled practitioner and leader who oversees daily operations in Continuing Care, managing LTC flow, patient placement, and admission and discharge processes. They complete assessments, plan and evaluate care, and collaborate with patients, families, and interdisciplinary teams to support safe, effective, patient- and family-centred outcomes. The Coordinator mentors staff across acute, community, and Continuing Care settings, offering real-time coaching to support accurate LTC eligibility, appropriate SSB referrals, and effective discharge planning. They model professional communication, collaborative problem-solving, and consistent application of program criteria. The role also delivers informal and formal education through bedside teaching, huddles, presentations, written tools, and orientation support to ensure understanding of SSB, TCI, and LTC pathways, strengthening clinical practice and system navigation.

Experience

  • Previous supervisory experience preferred
  • Minimum 5 years recent acute nursing experience

Education

  • Bachelor of Science in Nursing (BScN)

Other Education and Training

  • Current CPR Level C

Knowledge and Abilities

  • Proficiency with clinical documentation systems
  • Proficient in referrals, assessment, counseling, and care planning 
  • Strong organizational and prioritization skills in an autonomous environment

  • Ability to coordinate complex patient flow, admissions, transfers & discharges
  • Knowledge in aging/geriatric disability/illness issues
  • Ability to lead, mentor, and support staff in a collaborative team environment

Other Information

  • Registered/eligible with the College of Registered Nurses of Saskatchewan (CRNS)
  • Previous Coordinator experience is an asset.
  • Minimum three (3) in geriatrics and/or complex care is an asset
  • 1 year clinical experience with medically complex older adults is an asset
