Job Description: Reporting to the designated Manager, the Clinical Care Coordinator is responsible for ensuring the highest standard of care and services provided to the patients of the Yorkton Regional Health Centre. As an experienced practitioner, the Clinical Care Coordinator acts in a leadership role to provide specialized clinical expertise, coordination, teaching, and support to the patient, family members and interdisciplinary team members within a collaborative environment. The Clinical Care Coordinator has the direct authority to organize, direct, coordinate and supervise the work of the specific clinical area in order to achieve the provision of quality patient care. The Clinical Care Coordinator works closely with the Manager of the unit in program development, and the development of policies and procedures to ensure a consistent approach within the region. The Clinical Care Coordinator is required to provide coverage for other clinical coordinators as needed.
