Job Description: The Chronic Condition Nurse integrates knowledge, skills and the principles of teaching and learning to support healthy lifestyle choices and chronic disease prevention, education and management services for clients, families, and the general public and health professionals. The Chronic Condition Nurse uses a consultative approach, accepted standards and guidelines and current research findings to provide support to clients and health care professionals as appropriate.
The Chronic Condition Nurse is an integral part of the Primary Health Care Team. The employee works relatively independently and is expected to make decisions based on sound assessments in accordance with policies of Population and Public Health Services, the health region, the SRNA Standards of Practice and other relevant legislation and standards.
The Chronic Condition Nurse develops relationships with, and is supported by, specialty programs that manage chronic conditions.

Experience
- 3 yrs exp chronic disease/complex illness edu. or chronic disease mgnt required
Education
- Bachelor of Science in Nursing (BScN)
Licenses
- Certified Diabetes Educator (CDE)
Memberships
- Diabetes Education Section (DES)
Other Education and Training
- Nursing diploma is acceptable
- Nephrology Certification is desired
- Basic Cardiac Life Support Certificate (BCLS)
- Comparable degree in education/adult learning may be considered.
- Diabetes Education certificate (CDE) is acceptable
- Certified Diabetes Educator, is an asset
