Job Description: Reporting to the designated Health Services Manager (HSM)/Health Service Administrator(HSA), the Clinical Care Coordinator (CCC) is responsible for ensuring that the highest standard of care and services are provided to residents/clients. As an experienced practitioner, the CCC acts in a leadership role to provide specialized clinical expertise, coordination of resident care, and support and teaching to the resident care team (including residents, resident's family members, physicians, pharmacists~ nursing and all other facility staff) within a collaborative environment. The routine procedures are generally predetermined but critical thinking and independent judgment is an integral part of the position.

Education

• Bachelor of Science in Nursing (BScN)

Licenses

• Basic Life Support Certificate (BLS)

Other Education and Training

• Bachelor of Psychiatric Nursing is acceptable
• Nursing diploma is acceptable
• Psychiatric Nursing diploma is acceptable
