Job Description: The Patient Care Coordinator, under the direction of the Director of Acute and Clinical Services, is primarily accountable for the system navigation and coordination of patient flow in the health care centre. This is achieved by the Patient Care Coordinator having an ongoing awareness and involvement in the workflow of the health care centre. Duties include bed management, discharge planning, advocating for the patient/family, identifying supply and equipment needs, assisting with staff scheduling, supporting, coaching and mentoring staff, working collaboratively with other departments and staff on an as-needed basis, carrying out such audits as are deemed necessary to ensure that all activities are being done using Evidence Based Best Practices and Keewatin Yatthe Regional Health Authority Policies and Procedures, mission, vision, goals and values, acting on recommendations made by the Quality Performance and Improvement Committee and other Patient and Family Centred Care Committees such as the Patient Safety Committee, the Infection Control Committee and the Occupational Health and Safety Committee.

• Bachelor of Science in Nursing (BScN)

Licenses

• Basic Life Support Certificate (BLS)

• Bachelor of Psychiatric Nursing is acceptable
• Nursing diploma is acceptable
