Job Description: The Nurse Navigator, Kidney Health plays a crucial role in supporting patients navigating dialysis treatment and vascular access options. This position is responsible for promoting goals related to independent dialysis and body access, collaboration, with medical and inter-professional teams, inpatient teams, Kidney Health partners and other stakeholders. The Navigator facilitates a seamless patient journey during transitions, ensuring timely education on dialysis treatment options and coordinating services for optimal patient wellbeing. The Nurse Navigator works closely with key stakeholders to ensure patients receive comprehensive education on all aspects of dialysis treatment options enabling the patient to make an informed choice regarding dialysis treatment and vascular access.

Experience

  • Successful Completion of "Care of the Patient with Kidney Disease" Course
  • Five (5) years experience in a related area
  • Experience in program development

Education

  • Serves as an educator and resource person for renal patients.
  • Creates and implements standardized patient education and assessment tools
  • Assesses patients with End-Stage Renal Disease (ESRD) to ensure patients can make an informed dialysis treatment and access choice decision.
  • Assesses the patient’s understanding of renal disease and treatment options at time of admission to Hemodialysis Program.
  • Assesses learning needs of patients regarding their knowledge level of their disease progression
  • Ongoing assessment of ability and willingness to learn about chronic disease self-management and treatment options as well as well as identification of potential barriers for patient success relating to these choices.
  • Utilizes principles of adult learning, chronic disease management and motivational interviewing strategies in all communications with patients and families. Delivers continuous education & support throughout the hemodialysis journey to aid patients and their families in making care decisions that honor their individual values, beliefs, preferences and health goals.
  • Actively share your expertise and institutional knowledge with your colleagues. Documenting processes, best practice and insights will contribute to a smooth transition for those who may step into your role in the future.

Other Education and Training

  • Graduate from an approved School of Nursing.
  • Baccalaureate Degree in Nursing is required.
  • Nephrology certification is desired.

Experience

  • Minimum of three (3) years Hemodialysis related experience
  • Successful Completion of the “Care of the Patient with Kidney Disease” Saskatchewan Polytechnic Course
  • One (1) year of experience in adult education or a teaching role.
  • Completion of SK poly NURS-1680 Care of Patient with Kidney Disease
  • Requires experience in program/resource development related to needs assessment, planning, program design, implementation, and evaluation.

  • Teaching: Demonstrates the ability to transfer information through the principles of adult education.
  • Critical Thinking: Integrates and evaluates data from multiple sources to problem-solve effectively.

  • Current registration with the College of Registered Nurses of Saskatchewan (CRNS).
  • Current BCLS certification.
