Job Description: Under the leadership and direction of the Director - Five Hills Access Centre, the incumbent is responsible to act as the primary liaison between physicians, facility nursing unit managers, facility health care providers, and Continuing Care services (Home Care, Transition Programs - respite, waiting placement, convalescence, palliative care, long term care, Geriatric Assessment and Rehabilitation Unit, Adult Day Program) The Assessor Coordinator facilitates the transition/ discharge planning process from the acute care facility back to the community or Long Term Care in accordance with SHA policies, procedures, and guidelines. "Preference for this position will be given to HSAS members in accordance with the terms and conditions of the SAHO/HSAS Collective Agreement"

Licenses

  • Valid Class 5 driver's license
