Job Description: Under the direction of the Manager and as part of the interdisciplinary team, the Assessor Coordinator is responsible for case management services in an acute care area. Case management is provided to patients receiving services from Home Care, long term care and residents in personal care homes. Core functions include referral/intake, engagement, assessment, care planning, monitoring, evaluation, termination/discharge in collaboration with family, care givers, and multi-disciplinary team members. This position liaises with other area and provincial services. The position reports to a manager and a senior position(s) for daily clinical supervision. This position collaborates with acute care, rehabilitation, home care, long term care, mental health, government and non-government agencies to assess and coordinate services to meet needs. "Preference for this position will be given to HSAS members in accordance with the terms and conditions of the SAHO/HSAS Collective Agreement."

Experience
- One (1) year post grad experience in the specific department/area/program
Education
- Baccalaureate of Social Work degree
Licenses
- Valid Class 5 driver's license

- Bachelor of Nursing is acceptable

Other Information
- Registered/eligible with the Saskatchewan Association of Social Workers (SASW)
- 18-months of recognized clinical experience (post-graduate) preferred.
- Some assignments may require access to a vehicle for work.
- Preference to HSAS members as per the SAHO/HSAS Collective Agreement
