Job Description: The Assessor Coordinator-Degree Senior assists the Home Care Manager ensuring efficient, effective, coordinated delivery of home care services. This position ensures high quality client care using the established philosophy, objective, policies, and procedures. The incumbent uses collaborative case management, to ensure the client has access to appropriate, adequate, high quality, coordinated and integrated service. The incumbent uses a collaborative client centered process respecting the client's dignity and right to make informed decisions, family's role, caregiver's and community resources. The incumbent promotes quality care and cost effective outcomes, to address the client's health/well being by assessing, planning, coordinating, implementing, monitoring and evaluating services. This position has staff supervisory responsibilities. "Preference for this position will be given to HSAS members in accordance with the terms and conditions of the SAHO/HSAS Collective Agreement"

Education
- Diploma/Degree in Nursing or BSW, Physiotherapy, or Occupational Therapy Degree
Licenses
