Job Description: Under the direction of the Manager, and as part of an interdisciplinary team, the Assessor Coordinator - Degree - Senior is responsible to provide a complex range of screening, assessment, treatment and case coordination services to the community mental health and addictions population seen at the Urgent Care Center . The incumbent will provide consultation and coordination of service including prioritizing and referring to community agencies . Also provides evaluation of clinical interventions, supervision and professional and public education regarding mental health issues and resources . The Assessor Coordinator - Degree - Senior will provide leadership and clinical expertise in the development and maintenance of community mental health and addiction treatment programs . The Assessor Coordinator - Degree - Senior, along with community and the Urgent Care Center teams, provides crisis intervention, support, assessment and follow-up plans to clients in the community who are in crisis or require assistance. The Assessor Coordinator - Degree - Senior also assists in the successful stabilization and /or transition of acute patients from UCC to hospital/detox or alternatively to prevent admission/readmission by providing short term acute mental health/addiction services to those who present in crisis. Follow-up may include brief intervention/support, referral to appropriate community organizations/support networks or collaboration with existing supportive organizations. Prevention of ongoing or future crisis is a goal. Services are provided 24/7.

Human Resources Exemption: No

Experience

  • Min. 2 yrs (post grad) work exp. in community mental health/addiction services

Education

  • Recognized Degree in Nursing, Social Work or Addictions Counselling

Other Education and Training

  • Bachelor of Addictions Counseling is acceptable
  • Post-graduate training or courses in psychiatric nursing is an asset
  • Bachelor of Social Work is acceptable
