Social Worker

As a member of a multidisciplinary health care team, the Social Worker provides direct patient care through the provision of professional services in psychosocial adjustment/coping in physical rehabilitation which includes: screening, assessments and counselling; crisis intervention; bereavement counselling; liaison and referral to community agencies; advocacy and discharge planning. The Social Worker will provide Social Work services to patients who have experienced spinal cord injuries/limb amputation. 

1. Current Certificate of Registration with the Ontario College of Social Workers and Social Service Workers as a Social Worker
2. BSW with 3 to 5 years of experience in a health care setting; Masters degree preferred
3. Demonstrated knowledge and 2 recent years therapeutic experience within the last 5 years with clients who have had an amputation/musculoskeletal injury new
4. Demonstrated skills in screening, assessment, counselling, case management and advocacy skills
5. Expert knowledge of community resources
6. Demonstrated knowledge and 2 recent years therapeutic experience within the last 5 years in the field of neurological rehabilitation with Spinal Cord population new
7. Strong interpersonal, communication, teamwork, consultation and teaching skills
8. Demonstrated commitment to ongoing learning
9. Strong commitment to interdisciplinary care
10. Previous Rehabilitation experience preferred
