Description
The Social Worker will provide comprehensive clinical social work services to patients and their families working collaboratively with multidisciplinary disease site teams, as well as other Supportive Care professionals. Clinical activities include comprehensive individual and family assessments, counselling and therapy, and may include the requirement to lead psycho educational and/or therapeutic patient groups. This role also includes the provision of practical assistance, patient education and community referrals and liaison. Social workers at the JCC also participate in staff and public education, psychosocial oncology research initiatives and supervise Master's level students.
1. Masters Degree in social work from an accredited University
2. Certification as a Social Worker with the Ontario College of Social Workers and Social Service Workers
3. Minimum of 3 to 5 years of clinical social work practice involving individual and family counseling and/or therapy
4. Oncology and Counselling experience preferred
5. Experience working with individuals and families facing chronic and/or life threatening illness, loss/grief and adjustment issues
6. Demonstrated expertise in interviewing, the completion of comprehensive psychosocial assessments and the development of a plan of action
7. Excellent communication, interpersonal, teamwork and crisis intervention skills
8. Demonstrated skill and ability in the provision of health professional consultation and education
9. Demonstrated ability to identify and work with appropriate community resources to address patient and family needs
10. Demonstrated ability to organize, prioritize, problem solve and respond to competing, unscheduled demands
11. Familiarity with the Mental Health Act, Consent to Treatment Act, Substitute Decisions Act and Child and Family Services Act
11. Experience and/or willingness to participate in research and program and professional planning initiatives
