Social Worker MSW

The Social Worker is an integral member of the multidisciplinary care team and is an active member of the health care team. The Social Worker provides direct patient care through the provision of psychosocial screening and assessments; individual and family counseling and treatment; crisis intervention; bereavement counseling liaison and referral to community agencies; advocacy and discharge planning.

1. Current Certificate of Registration with the Ontario College of Social Workers and Social Service Workers as a Social Worker
2. Master’s degree in Social Work from an accredited School of Social Work
3. Recent (within 2 years) critical care experience and working with the outpatient population experience preferred
4. Knowledge of the Healthcare Consent Act and capacity issues
5. Demonstrated knowledge of child and family community support services
6. Knowledge and experience with WSIB
7. Demonstrated skills in the performance of a therapeutic role including strong assessment, counseling, case management, crisis intervention and advocacy skills
8. Knowledge of how to facilitate the use of PHQ9, Bio-Psych Social Assessment, ,GAD7, PTAS Scale, PTSD Screening, ATSD, PTSD
9. Proven ability to work in a fast-paced, complex environment and to organize, prioritize and problem-solve effectively
10. Strong interpersonal, communication, teamwork, consultation and teaching skills
11. Previous experience with the burn patient population
12. Demonstrated commitment to ongoing learning
