As a member of the healthcare team, the Registered Practical Nurse (RPN) - New Patient Referral provides support to the Juravinski Cancer Centre (JCC) patients, families, and members of the healthcare team. Reporting to the JCC Manager Flow and Operations, the RPN responsibilities include but are not limited to: working in the clinical area with newly diagnosed oncology patients.  This role will provide psychosocial support to the patients and families as they start their illness trajectory.  The RPN in the clinics will be completing a physical assessment of patients inclusive of interventions such as dressing changes, assisting with biopsies and other hands on activities as required such as injections and drain care.  In addition, the RPN will work within the referral office, and receives patient referrals for the JCC; evaluates and triages each referral to determine placement with the appropriate clinical team; schedules patient appointments; reviews, analyzes and documents referral information including cancer related health histories, demographic data, and clinical investigation reports. This role will also work within the outpatient oncology clinics supporting the interprofessional team within the scope of the RPN (i.e. Health history, assessments, best possible medication history, care coordination and psychosocial support to patients and families). The RPN works collaboratively with the interprofessional teams inclusive of Registered Nurses, Specialized Oncology Nurses, Advanced Practice Nurses, Disease Site Team Coordinators, physicians, allied health and with the New Patient Registration Clerks.


Qualifications
  • Current certificate of registration with the College of Nurses of Ontario as a Registered Practical Nurse and eligible to practice as a RPN in Ontario.

  • Minimum 3-5 years nursing experience in a hospital/outpatient setting.

  • Recent (within 2 years) Oncology nursing experience.

  • Demonstrated skill and ability in management and triage of patient referrals.

  • Membership with Canadian Association of Nurses in Oncology considered an asset

  • Current certificates - Physical Assessment and Health History Taking (or equivalent) Recent (within 2 years) patient assessment course or related experience demonstrating competent patient assessment skills and ability in health history taking

  • .Demonstrates critical thinking and decision making in assessment and problem identification and care planning. Demonstrated skill and ability in management and triage of patient referrals

  • Demonstrated ability to function within an interprofessional team, including nursing collaboration with RNs and other members of the healthcare team.

  • Demonstrated ability to function in a fast paced and at times complex environment.

  • Demonstrated ability to communicate (written and verbal) effectively with patients, families, and other members of the healthcare team.

  • Demonstrated ability to assess concurrent demands, set priorities, organize work, and maintain accuracy and concentration in periods of high activity in response to patient and team needs.

  • Demonstrates ongoing personal professional education and development

  • Completion of an entry level approved foundational course specializing in oncology, or willingness to enroll within first 30 days.

  • Demonstrated computer proficiency and experience with EPIC (registration and scheduling), MOSAIQ, Clinical Connect, MS Office and teletracking

  • Keyboarding of at least 45 wpm (subject to testing)
