Description
As a vital member of the healthcare team, the physiotherapist coordinates, assesses and evaluates patient care and provides physiotherapy care to meet the needs of vascular surgery patients. The physiotherapist collaborates with interdisciplinary team members in providing Phase 1 rehabilitation and education to patients and families. The physiotherapist functions as an integral member of the Physiotherapy Cardio-Vascular team providing services to 4 West and CCU.
The physiotherapist is proactively involved in discharge planning collaborating with interprofessional team members and community agencies. The physiotherapist practices in accordance with the College of Physiotherapist of Ontario Standards of Practice and Guidelines and according to Hamilton Health Science Policies and Procedures. The physiotherapist utilizes a patient and family centered care approach and incorporates best available evidence into practice.
The physiotherapist participates and provides clinical education to physiotherapy students and others through preceptor, tutor and/or clinical lecturer activities. In addition to providing clinical care, the physiotherapist actively participates in continuing professional development opportunities, professional practice and program committees and/initiatives that support patient care clinical practice, education and research as approved by Clinical Manager in conjunction with the Chief of Practice
1. Graduate of an approved university graduate level physiotherapy program (BScPT, MSc. PT) or equivalent
2. Current Registration in good standing with the College of Physiotherapists of Ontario
3. Well developed physiotherapy assessment and treatment skills relevant to acute care cardio-vascular surgery and
acute care cardiology patient populations
4. Demonstrated excellent communication, interpersonal skills and effective discharge planning skills and knowledge of
community resources and supports
5. Demonstrated ability to work independently while collaborating within an interdisciplinary healthcare team
6. Participation in continuing education in order to upgrade skills and keep abreast of new developments within cardiovascular
and cardiology clinical practice areas
7. Demonstrated ability to work in a changing environment with high productivity, organizational and flexibility demands
with solid clinical reasoning and caseload management skills
8. BCLS or Equivalent
