Description
The Occupational Therapist (OT) provides screening for eligibility for the Centralized Care and Transitions Team and works collaboratively with CCaTT team members to complete assessments, interventions and development of care plans for patients over 65 years of age at risk of deconditioning during their Emergency Department (ED) visit or hospital admission. Positions will be primarily focused in the ED and the Express unit. The OT participates as a member of an interdisciplinary team. The OT practices in accordance with the standards and guidelines for professional practice as per the College of Occupational Therapists of Ontario (COTO) and according to Hamilton Health Sciences policies & procedures. The OT incorporates best available evidence into practice.
In addition, the OT will actively participate in continuing professional development opportunities, professional practice and program committees & initiatives that support patient care, clinical practice, education and research as approved by the clinical manager.
Qualifications
1. Current registration in good standing with the College of Occupational Therapists of Ontario (COTO) 2. Experience working in an interdisciplinary team
3. Knowledge of outcome measurement and evidence-based practice
4. Demonstrated ability to work in a changing environment with high productivity, organizational and flexibility demands with solid clinical reasoning and caseload management skills.
5. Must have excellent communication and collaboration skills, and the ability to work within interdisciplinary teams across the hospital.
6. Familiarity with the Barthel Assessment Tool an asset
7. Minimum of 3 years experience in General Medicine and Geriatrics
8. Demonstrated knowledge, skills and abilities to develop individualized coordinated care plans for seniors living with multiple chronic diseases, frailty, cognitive impairment, etc
9. Ability to perform well in a fast-paced environment
10.Demonstrated expertise in complex geriatric assessment and rehabilitative approaches
11.Demonstrated current knowledge of and experience with discharge planning and community resources
12. Ability to creatively problem solve and facilitate change.
