Intake Coordinator

The Intake Coordinator will oversee the intake process for the Hospital Elder Life Program. As an integral member of the Specialized Geriatric Services team, the Intake Coordinator will work closely with in patient units across all HHS sites. Their key role is to manage referral intake, gathering demographic and clinical information to triage the referral, and develop individual patient delirium prevention and management and functional decline care plans. The Intake Coordinator will communicate with health care providers including physicians, clinical teams, volunteer resources, and patients and family members to ensure a cohesive care strategy. The Intake coordinator will be responsible for mentoring, organizing and assisting students and volunteers to deliver delirium prevention and management and functional decline interventions to patients and their care partners.   

1. Baccalaureate Degree in a health related field
2. Working knowledge of regional geriatric services
3. Familiarity with medical terminology
4. Demonstrated excellence in MS Word, Excel, Access, Exchange/Outlook, Schedule
5. Demonstrated excellent organizational skills
6. Demonstrated ability for independent problem-solving
7. Demonstrated appreciation of the confidentiality of the role
8. Demonstrated interest in continuous learning
9. Excellent interpersonal skills and verbal/written communication skills including telephone and email
10. Ability to prioritize work independently 
11. Member in good standing of a regulatory college.
