Description
The HHS at Home Navigator will support a unique model of service delivery centred on patient needs and facilitates transition to the most appropriate services that meet the individual and functional goals of the patient. The HHS at Home Navigator works in collaboration with the health care teams across the hospital, the service provider organization (home care delivery team) and other community partners to ensure patients can smoothly transition home with supports in a safe and timely manner. The Navigator provides education and support to HHS teams, patients and their families about the program and works to creatively address barriers to patient flow. The HHS at Home Navigator also takes the lead on evaluating and improving the program to ensure optimal performance and outcomes. The Navigator will be based at the Hamilton General Hospital site.
- Post secondary education; Degree in a relevant field
- Regulated Health Professional with current registration to applicable Regulatory College
- Recent, minimum 3-5 years’ experience in clinical practice, acute care and/or chronic disease management where participated in and facilitated effective patient transitions out of hospital
- Demonstrated knowledge and experience in working with a continuum of services across sectors (including home care), and resources to enhance patient self-management and quality of care preferred
- Demonstrated knowledge of community support services and homecare services and applicable legislation including the Connecting Care Act and Connecting People to Home and Community Care Act preferred
- Demonstrated excellent verbal and written communication skills and strong ability to build relationships and work both independently and collaboratively with various partners across the system
- Assertive problem solving, decision-making and conflict resolution skills
- Demonstrated excellence in consultation, negotiation, mediation and client advocacy
- Demonstrated experience in using adult learning principles to educate patients, families and other health care providers
- Skilled in using computer programs, including, Microsoft Excel, Word, Outlook, and Power Point.
- Knowledge and experience in leading and evaluating quality improvement initiatives using quality improvement methodologies
- Demonstrated commitment to personal learning and growth
