Responsible to the Director of Community and Population Health Services, the SHF Clinical Managers have leadership and managerial responsibilities at both the planning and operational level. The Clinical Managers are expected to design, lead, plan and evaluate improvement efforts for programs within the Community & Population Health Services portfolio, while developing and supporting diverse teams of staff caring for patients at the Satellite Health Facility. The Satellite Health Facility (SHF) is a 120 bed facility established to support patient flow across Hamilton Health Sciences sites. The patient population at the SHF consists of low acuity and ALC patients.


Qualifications

1. Regulated Health Professional with current certificate to practice.
2. Masters Degree in related field preferred.
3. Minimum 5 years formal managerial experience in acute hospital setting.
4. Demonstrated ability and knowledge to access and utilize hospital, primary care, home care and community resources.
5. Demonstrated knowledge of population health management frameworks.
6. Demonstrated evidence of continuous education and life long learning.
7. Demonstrated commitment to the mission, vision, and values of HHS.
8. Proven leadership abilities and effective communication, facilitation and presentation skills.
9. Demonstrated ability to facilitate quality improvement teams, apply quality improvement methods and tools, and coach/
mentor staff on the same.
10. Skill in supporting and mentoring staff to enhance knowledge, skill, and judgment within their practice.
11. Demonstrated leadership and coordination skills in a fast-paced environment.
12. Demonstrated creativity and innovation in the delivery of patient care services.
13. Demonstrated excellence in communication, team-building, and ability to initiate, model and support change in a
constructive manner.
14. Demonstrated advanced critical thinking skills.
15. Demonstrated skills in data collection design, collection of data, writing reports and analysis.
16. Demonstrated use of various data sources (eg. Canadian Institute of Health Information (CIHI) data base, Hay Report,
Workload, Canadian Inpatient Surveys, Quality of Life Surveys).
15. Proficiency in use of office support software (Word, Excel, PowerPoint, and Access).
