Description
The Clinical Leader for Community & Population Health Services Program is directly accountable to the Clinical Manager
of the AHF, a non-traditional hospital setting for patients no longer requiring acute/hospital-level care.
The Clinical Leader, under the direction of the Clinical Manager is responsible for:
• managing the day-to-day issues related to patient care
• developing, coordinating, and consulting and liaison with Professional Practice to ensure day to day maintenance and problem-solving regarding professional standards
• optimizing staff relationships within interdisciplinary teams
• identifying needs of the unit in relation to staffing resources
• monitoring staff attendance
• assisting with retention, recruitment and performance management
• ensuring implementation of policies and procedures
• managing care delivery within a family centered care environment
• overseeing availability of supplies and equipment
Additionally, the Clinical Leader will:
• act as a role model for clinical excellence, continuous learning
• assist the Clinical Manager with measurement and evaluation within the program
• participate in budget analysis and implementation
• lead change effectively within the unit
• facilitate communication between departments, programs and services
• promote team spirit, cohesiveness, motivation and commitment to program goals
Regulated Health Care Professional in good standing with health professional regulatory body in Ontario
University degree in a health-related field
Minimum 5 years’ leadership experience working within a unionized healthcare environment
Demonstrated advanced critical thinking skills in the provision of patient focused care
Demonstrated recent knowledge of Alternate Level of Care (ALC), patient flow, discharge planning
Demonstrated recent knowledge of community resources including Ontario Health atHome
Demonstrated ability to work independently and collaboratively with an interdisciplinary team, patients, families, primary care and community services
Demonstrated leadership skills, including problem solving, critical thinking, conflict resolution and negotiation required
Excellent interpersonal, communication, organizational and decision-making skills required
Demonstrated expertise in quality improvement and change management methodology required
Demonstrated commitment to continuous education and learning for self and others
Demonstrated commitment to the mission, vision and values of HHS
Knowledge and understanding of legislation governing health care delivery
Fluency in English (verbal and written)
Computer literacy and basic proficiency required
