Reporting to the VP of Nursing Practice, Education and Community Health, the Chief of Interprofessional Practice is focused upon improving patient outcomes in collaboration with operational and medical program leadership, staff and physicians. This is accomplished by providing expertise to enhance professional practice, interprofessional performance and capacity in the delivery of care. The Chief is aligned to Vice President portfolios to support strategic directions related to practice that improves quality of care and safety at Satellite Health Facility, West Lincoln Memorial Hospital, St. Peter’s Hospital and Urgent Care Center which includes Adult Ambulatory Care and CcaTT.
Experience with chronic disease management services is essential to this Chief as he/she facilitates a number of changes related to models of care, scopes of practice and quality improvements. 
Key functions in relation to the interprofessional practice accountabilities: Ensuring the establishment, implementation and evaluation of standards of practice; ensuring the continuing advancement of the professional practice through education, research, empowerment of staff, and policy development; applying methods to appraise and design models of care and skills mix team design; leading, supporting or monitoring practice changes and accountability, keeping up with technology advancements that impact clinical practice (particularly electronic documentation systems), supporting the adoption of evidence and best practices, and contributing to methods that improve team communications and performance.
This Chief jointly manages advanced practice nurses and other advanced professional practice roles in regards to clinical performance, their support to advancements in nursing practice within HHS, and their participation in education and research activities. Some other operational management responsibilities may be assigned to the role.
Team work with the other members of Interprofessional Practice team is critically important for the benefit of HHS at large. For instance, many members of the portfolio are very active on projects associated with the HHS strategic plan initiatives, HHS wide practice committees, corporate committees etc.
 


Qualifications
1. Current certificate of registration in good standing with a Regulated Health Professional College. Physiotherapist or Occupational Therapist preferred
2.Baccalaureate or Master’s degree in a health discipline that includes health administration and leadership is preferred. Doctoral level preparation is preferred. 
3.Current experience in professional and team leadership, and at least 5 years management experience with progressive and leadership responsibilities.
 4. Thorough understanding of nursing roles and nursing work in addition to knowledge of other health professionals work and their collective impacts on patient outcomes with particular emphasis on complex chronic disease services. 
5. Demonstrated career growth and development in leadership roles with increasing accountabilities and influence on professional and interprofessional practice.
6. Demonstrated excellence in written and oral communication, critical thinking, analysis, consultation and team building skills. A proven record of building relationships and networks and navigating highly political issues.
7. Experience with provincial initiatives that improve services for patients and families with chronic diseases is required. 
8. Evidence of knowledge and experiences with changes that contribute to transformative care approaches that create more value to patients, quality and contribute ways to be cost effective and efficient. Experience with Lean methodologies would be preferred.
9. Evidence of past experience leading successful project teams where deliverables create a lasting and significant impact on improved clinical performance and improved patient care.
