Detailed Overview

Reporting to the Program Coordinator and receiving Clinical Social Work support from the Clinical Chief at Chilliwack General Hospital and as a member of the interdisciplinary team, the Social Worker provides comprehensive therapeutic mental health interventions and conducts comprehensive Social Work and psycho-social assessments of residents' personal support systems, social history, strengths and resources from a psychosocial rehabilitation (PSR) and recovery perspective an develops relevant rehabilitative programs and care plans for residents. Educates and provides support and consultation to residents, families and healthcare professionals. Develops, coordinates and delivers in-services, presentations, and psycho-educational and support groups in collaboration with the interdisciplinary team. The Social Worker is expected to engage the resident and their support systems as people with full competence and as full collaborators in service planning, delivery and evaluation.
Responsibilities


  1. Provides comprehensive therapeutic mental health intervention to residents focusing on the family, social, emotional, and cultural needs of the residents; provides psycho-education to residents and their families;develops,implements,evaluates and documents psycho-social rehabilitative plans including life skills training in the areas of personal life, leisure, education and work, in collaboration with the resident, family and interdisciplinary team members.
  2. Conducts comprehensive Social Work and psycho-social assessments of residents by methods such as interviewing the resident and/or family, obtaining relevant information, gathering social data regarding the patient and/or family and interpreting the data using a variety of modalities in accordance with professional standards and clinical policies and practices. Provides continuous assessment of the resident's ability to perform activities of daily living by participating in both group and individual activities related to the realm of personal life, leisure, education and work.
  3. Works with other team members to screen referrals, and assess residents for admission to the facility; participates by taking a lead role in the admission, discharge and transfer of residents within the Tertiary Adult Rehabilitation Program.
  4. Assists in the development, implementation and evaluation of individual and/or group activity programs, rehabilitation programs or other programs. Works closely with other staff members with respect to implementation of the recovery centered clinical system. Engages in the recovery model of working along side residents and other disciplines in the planning and preparation of snacks/foods and light housekeeping duties. Liaises with community-based agencies to ensure support for residents.
  5. Determines the financial and vocational needs of residents and assists residents and families by working in coordination with community resources to meet these needs.
  6. Evaluates the effectiveness of Social Work interventions and communicates to the interdisciplinary team and the physician/psychiatrist of changes in the residents' condition; assists in the development, implementation and evaluation of resident and/or group activity programs, rehabilitation programs or other programs. Seeks clinical social work direction from the Chief of Social Work as necessary.
  7. Participates in interdisciplinary activities such as resident conferences and daily/weekly report meetings.
  8. Transports residents to programs, appointments, services and other activities; books client trips, alters schedules, arranges time for pickup and drop off of residents.
  9. Maintains patient records by methods such as documenting patient assessments, charting patient information, preparing progress notes, discharge summary and other reports in accordance with established standards. Completes workload statistics, program records and reports for submission to Program Coordinator or Clinical Chief of Social Work as required.
  10. Develops, coordinates and delivers in-services, presentations, psycho-educational and support groups and/or training related to mental health by methods such as collaborating with team members of the interdisciplinary team, determining relevant content and required audio-visual/handout aids, conducting literature searches and planning and coordinating seminars.
  11. Participates in student education by providing guidance and input to current and potential Social Work students.  
  12. Maintains current knowledge in social Work practice in the mental health field by methods such as reviewing literature, consulting with clinical and community experts within the field, evaluating clinical practice and participating in professional development activities in order to improve care through new practices.
  13. Performs other related duties as assigned.

Qualifications

Education and Experience

Bachelor Degree in Social Work from an accredited School of Social Work, plus two (2) years recent related experience within the mental health care field. Current full registration with the BC College of Social Workers. Current CPR Level C and Current Food Safe Certificate. Valid BC Drivers License and access to personal vehicle for business use.

Skills and Abilities

  • Comprehensive knowledge and understanding of emotional issues related to mental illness and addictions and resident and family cultural dynamics in a tertiary adult rehabilitation setting.
  • Comprehensive mental health intervention skills including crisis counseling, brief therapeutic interventions and group work applicable to resents and families.
  • Knowledge of psychiatric disorders, medications and side effects.
  • Working knowledge and application of psychosocial rehabilitation and recovery principles.
  • Ability to make effective referrals, reflecting both in-depth knowledge of community resources and supports.
  • Ability to educate and provide consultation to residents, families and healthcare professionals.
  • Ability to utilize outcome measurement tools.
  • Ability to assess situations and diffuse crisis incidents.
  • Knowledge of relevant legislation including the Mental Health Act.
  • Ability to initiate, develop and carry out research activities.
  • Ability to organize work and prioritize workload.
  • Ability to communicate effectively both verbally and in writing.
  • Physical ability to perform the duties of the position.
  • Ability to work effectively with others in an interdisciplinary team environment.
  • Ability to operate related equipment.
