Detailed Overview

Supporting the Vision, Values, Purpose and Commitments of Fraser Health including service delivery that is centered around patients/clients/residents and families and in accordance with the British Columbia College of Nurses and Midwives (BCCNM) standards of practice, the Transcranial Magnetic Stimulation (TMS) LPN provides nursing care for clients before, during and after undergoing TMS treatment for conditions that include treatment resistant depression, both by working independently and as a member of an integrated care team. Prepares client for treatment, assists with the informed consent process, administers the TMS treatment and/or provides nursing assistance to the physician administering the treatment, monitors client condition following treatment, and documents in accordance with organizational and professional standards. Supports smooth transitions to and from the clinic (Neurostimulation, Mental Health Centres, UCRC), organizes appointments, and liaises and coordinates treatment with various inpatient and community services. Participates on designated hospital committees/teams and approved research projects. May be required to travel to other sites as needed.


Responsibilities


  1. Provides individualized direct nursing care to clients who are undergoing TMS for conditions that include treatment resistant depression. This includes the performance of physical and psychosocial screening and assessments and participation in the development and revision of client treatment and interdisciplinary care plans in conjunction with clients, families, and providers.
  2. Advocates for and facilitates informed choices and consent. Provides education to clients/families about the purpose, risks, and benefits of TMS, implications and potential side effects of such treatment, client rights about withdrawing consent and the management and other treatment options for the specific medical conditions being treated.
  3. Establishes therapeutic relationships with clients/family and provides emotional support to clients by identifying client/family perspectives, learning needs and readiness and ability to learn. Initiates discussion to identify and incorporate client and family priorities, wishes and goals into the care plan. Monitors client progress to ensure that treatment goals are met within established time frames.
  4. Consults, confers, and collaborates with peers and other caregivers in the identification and resolution of client/family care concerns, consent and special population issues, ethical dilemmas, and coordination and integration of care. Collaborates with the interdisciplinary care team in rounds and client care conferences with the client/family. Acts as client advocate to protect and promote client's right to autonomy, respect, privacy, dignity, and access to information.
  5. Documents client/family perspectives, nursing assessments, educational information provided, and activities that support client/family priorities. As applicable, ensures documentation of pre-procedure assessment findings and valid informed consent. Maintains the client health record in accordance with established professional and organizational documentation standards. This includes documenting care, communicating risk, preparing reports to meet regulatory requirements, and providing evidence and rationale for client-centred care/treatment planning.
  6. Administers the TMS treatment(s) by using the TMS electronic device following client-specific orders and clinical treatment parameters specified by the physician, and monitors for the client response and/or side effects and escalates concerns appropriately.
  7. Uses appropriate supplies and equipment for client care considering cost, availability, and safe care requirements. Maintains equipment and problem-solves malfunctions in collaboration with technical staff. Handles, stores, and disposes of workplace hazardous materials and sharps in accordance with established procedures and policies.
  8. Identifies significant and/or life-threatening changes to client’s condition and escalates to Registered Nurse and/or Physician and intervenes to manage changes as directed. Evaluates outcomes of these interventions and initiates hospital response resources appropriate to critical events where appropriate. Acts to resolve problems or emergency situations in collaboration with Registered Nurse and/or Physician. Assesses client for transfer in accordance with established transfer criteria and provides systematic report to nursing staff in receiving unit.
  9. Responds to incidents such as injuries to staff or visitors, Code Blue/White emergencies, hazardous spills, fire and aggressive/disruptive behaviour to ensure medical and other support is accessed in a timely manner. Informs designated staff of occurrences and completes incident reports in accordance with established procedures and policies.
  10. Conducts psychosocial assessments and works with the multidisciplinary team to provide trauma-informed, culturally safe, recovery-oriented and evidence informed care to clients exhibiting impaired function related to mental and emotional distress and crisis which may include alteration in thinking and self-concept, behaviours that indicate the client is a danger to self or others, monitoring for side effects/toxicities associated with psychopharmacologic interventions (if utilized), and escalates care to Registered Nurse and/or physician as required.
  11. In collaboration with the Physician, screens referrals to TMS treatment by applying inclusion and exclusion criteria. Communicates referral status with referring provider. Facilitates discharge planning and referral to community services.
  12. Provides support with appointment scheduling and coordination. In collaboration with Registered Nurse and/or Physician, prioritizes emergent situations with routine procedures in response to constantly fluctuating workload volumes and client treatment requirements in consultation with team members.
  13. Participates in program development through methods such as attending meetings, drafting or revising client care plans for review by program leadership and providing input to promote improvements to client and family care and the efficient and safe operation of the unit as required.
  14. Guides the clinical experience of assigned students and new staff and helps to orient new hires to unit policies and procedures.
  15. Sets measurable performance goals in collaboration with the Manager for professional development such as updating own knowledge within area of practice by reviewing current literature and research, identifying new theories, trends and opportunities, and developing a plan in collaboration with the Clinical Nurse Educator and Manager for professional development.
  16. Applies a research approach to clinical problem solving through methods such as, reviewing relevant literature/resources, and applying research findings within established procedures and/or collaboration with Clinical Nurse Specialist, Operations Manager, Coordinator, or Clinical Nurse Educator. Evaluates results.
  17. Participates in approved nursing and interdisciplinary research and special projects in collaboration with other health care professionals. This can involve collecting data, integrating and interpreting data, summarizing information, and/or providing updates for review and further analysis by principal researcher, research team and/or project coordinator.
  18. Performs other related duties as assigned.

Qualifications

Education, Training & Experience

 

Current practicing registration as a Licensed Practical Nurse with the British Columbia College of Nurses and Midwives (BCCNM).

 

Successful completion of additional education in Mental Health & Substance Use as per the BCCNM limits and conditions for licensed practical nurses.

 

Successful completion of additional training in Transcranial Magnetic Stimulation (TMS).

 

Two (2) years’ recent experience providing acute or community health nursing care/services, or an equivalent combination of education, training, and experience.

 

Skills & Abilities

  • Demonstrated ability in nursing practice related to clients with psychiatric illness who are undergoing TMS.
  • Demonstrated knowledge of concurrent disorders, substance use and abuse, harm reduction strategies including assessment and intervention.
  • Demonstrated knowledge of the principles and practices of conceptual models of psychiatric care including person and family centered care, recovery-oriented model in mental illness, and psychosocial rehabilitation methodology.
  • Demonstrated knowledge of the Mental Health Act as it applies to the rights and obligations of clients and staff.
  • Demonstrated skill in nursing within an autonomous scope of practice and adherence to standards of care relevant to designated client population.
  • Demonstrated skills in CPR techniques.
  • Demonstrated ability to participate in the development and delivery of educational programs and materials for clients and families based on education theories, principles, and best practices.
  • Demonstrated computer literacy to operate a computerized client care information system as well as applicable word processing, spreadsheet, and database software applications.
  • General knowledge of Trauma-Informed Practice and strengths-based care.
  • Demonstrated ability to promote Indigenous Cultural Safety.
  • Demonstrated ability to provide competent and culturally safe care in a variety of settings and with diverse populations.
  • Physical ability to perform the duties of the position.
