Detailed Overview
Reporting to the Manager, the Intake Worker screens, assesses and refers substance dependant adults and youth who call the Withdrawal Management Service seeking assistance with withdrawal management and problematic substance use. Coordinates admissions into the withdrawal management facility and other agencies; provides information to clients, families, health care providers, and the general public; maintains client's records; coordinates bed availability with direct and contracted agencies and client admission into available beds; maintains and updates waitlists. Maintains, compiles, and provides data and statistics, produces reports as requested, and compiles designated material for distribution.
Responsibilities
- Screens and assesses adults and youth by conducting telephone screening interviews according to established procedures, gathering client information/history and additional information. Determines appropriateness of request, client eligibility, and availability of resources. Consults with Manager/Client Care Coordinator/interdisciplinary team to determine best approach to care when required; provides information about housing, addictions residential services, and Withdrawal Management Services; enters client information into meditech such as medical and/or psychiatric diagnosis, current medications and substance use; refers to other community resources when appropriate.
- Assists individuals who call the Withdrawal Management Centre using listening and de-escalation skills, and determining appropriate response to crisis including referral to other facilities and immediate emergency response if required.
- Coordinates admissions, in consultation with the nurse in charge, into residential facilities and other appropriate community agencies by entering the required referral information into meditech, and by liaising with both the client and the applicable facility/agency in order to determine admission date and time.
- Provides information to clients, families, health care providers and the general public regarding various community resources including general resources (such as Emergency Shelters, free phones, food sources and refuge), harm reductions resources (such as Supervised Injection Site, Needle Exchange, pregnancy and STD testing, and other educational resources), and withdrawal services and treatment programs (such as Daytox, Community Health Centres and methadone maintenance treatment).
- Monitors bed availability within the Withdrawal Management Centre, consults with the Nurse in Charge several times daily to attain updated information on the number of beds open and the types of beds available.
- Maintains client electronic records by performing duties such as entering information into computerized client database including admissions, referrals, discharges, service eligibility, and forwards designated information to applicable facility or agency when required.
- Maintains and updates Addictions Services waitlist following established procedures for placement of hospital and community clients in relation to bed vacancies by methods such as contacting client to see if they still require to be waitlisted; adjusting placement needs and level of care required; responding to inquires regarding waitlist status; monitoring availability of resources; determining the appropriateness, ability and willingness to be referred to other addiction/mental health teams within Fraser Health according to availability and established procedures; recommending alternative sources of services; and consulting with Manager/interdisciplinary team to determine best approach when required.
- Maintains, compiles and provides data and statistics as directed regarding information such as waitlists, client admissions, length of stay, number of referrals, and nature of requests by recording information, entering into computerized systems, entering commands to generate reports as required for review by management team.
- Participates in quality improvement activities by attending meetings and in-services and providing input/recommendations to Manager and/or other team members regarding efficiency of processes/procedures relating to work duties.
- Performs other related duties as assigned.
Education and Experience
Diploma in Community Social Service from a recognized program and two years' recent related experience working with substance dependant adults and youth requiring withdrawal management services and assistance with problematic substance use, or an equivalent combination of education, training and experience. Valid CPR and First Aid certificates.
