Detailed Overview
Responsible for coordinating efficient and timely patient placement and bed utilization at designated Fraser Health (FH) acute sites; collaborates with external health care agencies regarding patient placement to and from FH acute sites; provides guidance to designated staff at the acute site to facilitate timely patient placement by assessing the efficiency and effectiveness of patient placement within applicable site(s) and surrounding regional and provincial centers; collaborates with other health care facilities and Patient Transfer Network regarding access to beds and acts as a resource to other service providers regarding access and flow issues; implements appropriate policies and procedures and provides guidance to health care teams regarding admission requirements and criteria; notifies the Manager of any actual or potential problems for resolution; maintains communications with staff, families and physicians on issues relating to patient placement, bed utilization and related information.
Responsibilities
- Facilitates timely patient placement at designated FH acute sites; coordinates patient placement activities by assessing current and projected capacity, prioritizing and expediting critical issues requiring immediate action, recommending bed openings and/or temporarily removing beds from service; leads daily bed management meetings with Patient Care Coordinators and evaluates potential staff implications to ensure optimal flow of patients between various units on any given shift.
- Consults with Patient Transfer Network, Community/Placement Supports as required to identify appropriate care for patients by developing and maintaining an understanding of available services at external health care facilities and agencies, determining appropriate contacts and discussing bed availability and resources on a patient by patient basis.
- Provides consultation and advice to FH leadership, acute and community staff on the interpretation and application of patient access and flow policies and procedures and eligibility for non-acute bed resources; assesses critical patient access and flow situations and makes recommendations on course of action related to bed utilization and patient placement.
- Acts as a resource and provides clinical guidance in patient placement and flow and bed utilization when there is difficulty in accessing resources for patient care; assists Bed Booking staff in allocating beds as required, interfacing with nursing units and programs and notifying the Manager of any actual or potential problems and follow up.
- Develops and recommends changes to current patient access and flow policies and procedures and drafts new and/or revised procedures, standards and criteria in conjunction with the interdisciplinary team to optimize timely bed access and appropriate patient placement.
- Analyzes statistics and trends (i.e., ALC, NICU, Perinatal) at the assigned site and identifies and participates in strategies such as Complex Discharge Rounds to best manage the designated patient population; works with clerical support staff on maintaining a database for acute bed availability to achieve optimal bed utilization and ensure patient information is efficiently managed.
- Participates in staff education program and assists in the development and provision of an orientation package for students, new staff and other health professionals.
- Participates on internal committees to provide an organizational perspective regarding patient placement and bed utilization and to provide an update of issues and trends and communicate opportunities for improvement and development.
- Represents FH on external committees and other project groups at local levels to ensure that FH interests are conveyed.
- Performs other related duties as assigned.
Education and Experience
Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
Three (3) years' recent, related clinical experience in an acute care setting, including one (1) year experience in supervisory and/or leadership role responsible for patient care coordination, access and planning, or an equivalent combination of education, training and experience.
Valid BC Driver's License and access to a personal vehicle for business-related purposes

- Demonstrated knowledge of relevant acts, legislation, standards and policies
- Demonstrated knowledge of professional theory and practice in working with clients, families, care providers, professionals and general public
- Abilioty to develop and maintain rapport with internal and external stakeholders
- Ability to operate related equipment including applicable software applications
- Physical ability to perform the duties of the position
