Salary range:

$88,600 to $106,300 per year plus extended health benefits and HOOPP pension plan.

Position Summary:

The Manager, Operations and Integrated Services will provide operational management of an integrated health and community service initiative - the Primary Care Action Team (PCAT) program. Reporting to the Director of Community Health or designate and working collaboratively with stakeholders across Flemingdon Health Centre, Ontario Health Teams and Primary Care Networks, the Manager will ensure the development and implementation of a high quality, integrated, interdisciplinary primary health care program. This position includes integration leadership, such as stakeholder management and system level strategy, operations leadership, such as project management and budget monitoring, with implementation services leadership, such as program delivery and managing staff.

This position focuses on leadership within a collaborative, client-centred and equity-focused framework. The successful candidate will be skilled in stakeholder engagement, project management best practices, and healthcare transformation.

Key Responsibilities:

Project Planning & Execution

  • Develop and design the PCAT program and oversee detailed project plans, timelines, and progress tracking.
  • Lead PCAT project team meetings, stakeholder sessions, and working groups.
  • Monitor PCAT project progress against goals, adjusting plans as needed.
  • Lead the development of robust program reporting frameworks to monitor progress, evaluate outcomes, and ensure accountability across all initiatives. This includes the use of performance indicators that reflect both clinical and community impact.
  • Champion data sharing and digital integration, working closely with internal teams and partners to streamline information flow, support digital health tools, and enable real-time access to data that informs care planning and system improvement.

Stakeholder Engagement & Communication

  • Facilitate communication across internal and external partners, ensuring all stakeholders are aligned and informed.
  • Champion a co-design approach by involving clients, families, and frontline providers in planning and evaluation.
  • Prepare PCAT presentations, reports, and project updates for leadership and working groups.

Program Implementation

  • Lead the planning and implementation of the Primary Care Action Team Program
  • Develop integrated primary care programs in community
  • Develop partnerships with other primary and community care partners
  • Monitor program budgets

People Management

  • Hire and supervise primary care staff including nurse practitioners, nurses, social workers, community health workers and community ambassadors
  • Work within FHC’s Collective Agreement

Change Management & Quality Improvement

  • Support change management activities including training coordination, communications, and feedback loops.
  • Apply quality improvement tools such as process mapping, logic models, and performance indicators.
  • Track outcomes and assist with evaluation efforts related to project deliverables.

Risk and Issue Management

  • Identify potential project risks and escalate them appropriately.
  • Recommend mitigation strategies and support problem-solving efforts within the project team.

Qualifications & Experience:

  • Bachelor's degree in health administration, Public Health, Business Administration, or related field.
  • Project Management Certification (PMP or equivalent) considered an asset.
  • Minimum 3 years of experience managing or coordinating complex projects in health care, social services, or related sectors.
  • Experience developing and/or implementing primary care programs
  • Strong organizational skills and experience developing and maintaining project documentation.
  • Demonstrated experience with stakeholder engagement and cross-sector collaboration.
  • Knowledge of the Ontario health care system and/or experience in integrated care models. Experience applying quality improvement frameworks and tools is considered an asset.
  • A clinical background is considered an asset.
  • Excellent interpersonal, written, and verbal communication skills.
  • Proficient with Microsoft Office Suite, project tracking software (e.g., MS Project, Asana, or equivalent) and Office 365.

Requested skills
