Position Summary:

Flemingdon Health Centre (FHC) is seeking a passionate and community-driven Project Coordinator to lead the Youth Violence Prevention (YVP) Project, funded by the City of Toronto. This initiative supports MVP youth (ages 10–29)—those most vulnerable to serious crime and violence—through co-designed programs focused on prevention, intervention, and interruption.

The Community Health Worker will work closely with youth, community partners, and advisory groups in Flemingdon Park to develop responsive programming that addresses barriers to basic needs, builds capacity, and fosters resilience. The goal is to empower youth to live, learn, work, and play in safe and nurturing environments.

*MVP - (youth most vulnerable to involvement in serious crime and violence)

Responsibilities include:

Program Development & Delivery

  • Assess the needs of youth vulnerable to violence in Thorncliffe and Flemingdon Park.
  • Design and deliver programs on health, relationships, violence prevention, and social development.
  • Apply a strengths-based, opportunity-focused approach to youth engagement.
  • Use the Collective Impact Framework to guide community change strategies.

Community Engagement & Partnerships

  • Build strong partnerships with schools, public health, housing, police, crisis response teams, probation services, and community leaders.
  • Conduct outreach and maintain a visible presence in Flemingdon and Thorncliffe Park.
  • Engage youth leaders and stakeholders to support and co-design programming.
  • Support advisory groups and youth involvement in planning.

Program Management & Evaluation

  • Coordinate consultations, reports, and presentations under the Manager, Population Health and Wellness.
  • Plan, implement, and evaluate youth health promotion programs.
  • Maintain work plans, budgets, and statistical records.
  • Supervise students, volunteers, and community participants.
  • Share program insights with internal teams.

Training & Capacity Building

  • Identify mentorship opportunities and support at-risk youth.
  • Facilitate educational seminars and training for youth and staff.
  • Participate in workshops, planning meetings, and professional development.

Administration & Reporting

  • Maintain accurate records and ensure timely reporting to FHC and the City of Toronto.
  • Ensure compliance with FHC policies and procedures.
  • Participate in internal and external committees and networks.

Skills and Qualifications

  • University Degree or completion of College Diploma in a related field such as Child and Youth Worker, Social Service Worker/Social Work, Social Science and/or an approved equivalent of education and related experience.
  • Minimum 3 years of experience working with youth in community-based or violence prevention settings.
  • Experience working with LGBTQ youth as well as utilizing a harm reduction approach with youth that have addiction issues is an asset;
  • Mental Health First Aid and Crisis intervention training (CPI, NVCI, TCI) considered an asset.
  • Strong understanding of equity, trauma-informed care, and systemic barriers affecting youth.
  • Proven ability to build relationships with diverse stakeholders and community partners.
  • Excellent communication, facilitation, and organizational skills.
  • Experience in program planning, implementation, and evaluation.
  • Familiarity with the Collective Impact Framework and youth engagement strategies.
  • Ability to work flexible hours, including evenings and weekends, as needed.
  • Knowledge of Flemingdon Park and Thorncliffe Park communities is an asset.
  • Fluency in additional languages is considered an asset.
