SUMMARY OF THE POSITION
A person who, within the scope of their specialty and in accordance with the legislation in effect, orients, assesses, provides primary care services and ensures the clinical follow-up of clients of all ages. The Nurse Practitioner exercises autonomously within their scope of practice: she assesses the physical and mental condition of clients, makes a diagnosis, prescribes diagnostic tests, medications and medical treatments, and performs invasive techniques in accordance with her reserved activities. She ensures the comprehensive and continuous management of the clientele, in partnership with the family physicians and the interdisciplinary team.
She provides clinical support to the members of her team, including nurses in extended role, licensed practical nurses, PCCRs, etc. She is called upon to contribute to the design, implementation and evaluation of programs related to her area of care. She participates in the development and implementation of teaching programs for clients and their families, when necessary. She may participate in the development of clinical documents, including collective prescriptions and protocols. She is involved in the development of advanced practice and contributes to nursing and medical teaching. She conducts or collaborates in nursing research and medical clinical research. Finally, she acts as a consultant, bringing her specialized expertise to bear on the health issues of the clientele, their families and the clinical decisions that follow.
As part of the deployment of the CBHSSJB’s new model of care, the Nurse Practitioner is called to play a provider role within integrated teams centered on the person, family and community. She actively contributes to the evolution of the care trajectory, to the strengthening of local capacity and to the alignment between clinical and community services, in an approach rooted in cultural safety and co-construction with the population of the territory.
SPECIFIC FUNCTIONS
Assessment and diagnosis
• Assess the physical and mental condition of clients of all ages presenting with common, acute or chronic health problems;
• Diagnose health problems that present with recognized diagnostic criteria and clinical manifestations;
• Prescribe diagnostic tests, interpret them and ensure their follow-up;
• Perform invasive techniques or techniques involving a risk of harm to confirm or rule out diagnoses, according to her competencies;
Treatment plan and follow-up
• Determine and prescribe pharmacological and non-pharmacological treatment plans, in accordance with evidence-based data;
• Prescribe medications and other substances, medical treatments and diagnostic or therapeutic techniques in accordance with the activities reserved by the OIIQ;
• Provide the required monitoring and follow-up during the management of a health problem, including control visits, triage and analysis of laboratory results and medical consultations;
• Manage abnormal results within her area of expertise; contact the partner physician when the situation exceeds her scope of competence or the legal framework of her professional practice, to jointly establish the appropriate course of action;
• Refer clients, on an urgent or elective basis, according to the clinical impression or diagnosis made;
Health promotion, prevention and community care
• Intervene to promote health and prevent illness, by supporting the person or a group in acquiring self-care skills;
• Participate in the organization of community activities aimed at health promotion and disease prevention, in collaboration with local teams;
• Adapt interventions to the cultural, social and contextual realities of Cree communities, in a respectful, safe and person-centered approach;
• Participate in nursing care provided to the community when the situation requires it (e.g., home visits, specific follow-ups);
Collaboration and clinical leadership
• Work in partnership with the family physician, nurses in extended role, Primary Care Community Representatives and other professionals of the interdisciplinary team;
• Participate in the development and revision of collective prescriptions, protocols and other clinical documents;
• Provide leadership in the professional development of nursing teams and contribute to the continuous improvement of quality and safety of care;
• Contribute to the training and education of PCSNP (Primary Care Specialized Nurse Practitioner) students and to the transfer of knowledge within teams.
REQUIREMENTS
Education:
• Master’s degree in Nursing Sciences;
• DESS in Primary Care;
• Holder of an IPSPL specialist certificate;
• Be a member in good standing of the O.I.I.Q).
Experience:
• One (1) year of experience as a Primary Care Specialized Nurse Practitioner is an asset;
• Experience in northern regions or in a First nation community is an asset;
• Candidate NPs are eligible.
Knowledge and Abilities:
• Knowledge of and experience with public and community health prevention and promotion related approaches, methods, issues (and in First Nation contexts) being an asset;
• Knowledge of MSSS programs and regulations for first lines services is an asset;
• Teaching abilities;
• Knowledge of Cree culture, and social/health issues in Eeyou Istchee is an asset;
• Knowledge of Eenou/Eeyou Pimaatsiiun healing practices and paradigms is an asset;
• Demonstrate interpersonal, leadership and teamwork skills;
• Excellent critical thinking, organizational and decision-making skills;
• Demonstrate independence, flexibility and empathy.
Language:
• Fluent in English;
• Fluency in Cree or French is an asset.
Other:
• Available to travel within the Eeyou Istchee territory as needed.
