SUMMARY OF THE POSITION
A person who is responsible for providing nursing care services according to the bio-psycho-social needs of users or groups under her/his care. She/he evaluates the health condition of users, as well as determines and implements nursing care and treatment plans. She/he provides nursing and medical care and treatment aimed at maintaining and restoring health, as well as preventing disease. She/he plans, provides and evaluates teaching activities intended for the users, their relatives and groups of people. In addition, she/he participates in research activities aimed at promoting health and preventing disease.

SPECIFIC FUNCTIONS
The liaison nurse plays a key role in coordinating medical services for clients who require care from 2nd and 3rd line inside and/or outside of their community. In close collaboration with Wiichihiituwin and specialized services departments, the liaison nurse is responsible for organizing and preparing medical visits for clients from Eeyou Istchee who will meet specialists and their follow-up/care when they return in the community. This includes reviewing medical consultation requests and reports. Acting as a vital link between the client, hospitals, and Cree communities, the liaison nurse ensures that relevant medical information is shared with the appropriate healthcare professionals in the community, including the care team and extended team to ensure continuity of care.

• Manage new consultation requests according to the defined service offer and service trajectories;
• Ensure validation of consultation forms and prerequisites before transferring request;
• Communicate with care team nurse to assure nursing care done before booking of the related consultation;
• Follow up on missing or expired documents (e.g., RAMQ) in collaboration with administrative staff;
• Use the digital platforms (e.g., Care4) to organize and manage the required documentation and track client records;
• Consider any delays, local medical follow-ups, and changes in the client’s medical condition that may occur while waiting for the specialist consultation, and communicate these to the specialist in collaboration with Wiichihiituwin and Specialized Services;
• Manage returns from medical visits, ensuring validation of medical reports and results;
• Notify the appropriate local care team nurse and other departments as needed of any new medical follow-ups or treatments required;
• Uses digital platforms (e.g., Care4 and MYLE) to organize and manage required documentation and track client files.

REQUIREMENTS
Education:
• Member in good standing of the O.I.I.Q .

Experience:
• Three (3) years of experience as a nurse.

Language:
• Fluent in English;
• Fluency in Cree or French is an asset.
