SUMMARY OF THE POSITION
Person who provides a variety of nursing services according to the bio-psycho-social needs of the users or groups of people under her/his care. She/he evaluates the health condition of users; determines and implements treatment and nursing care plans for users with complex health problems and/or problems that involve various biological, psychological and social aspects. She/he provides nursing and medical care and treatments aimed at maintaining and restoring health and preventing disease.

She/he conceives implements and evaluates health care programs that require advanced knowledge and that are aimed at solving complex health problems and/or problems involving various biological, psychological and social aspects. This person is involved in the development or adaptation of clinical and health care quality evaluation tools. She/he coordinates team work and; plays an advisory role towards her/his colleagues and the interdisciplinary team.

She/he identifies, for the users, the needs and interventions that require the coordination of different services within the establishment and between different establishments or related organizations. If applicable, she/he ensures the coordination of these services.

She/he is responsible for the orientation of new personnel and is involved in their training. She/he supervises teaching activities aimed at the users and their relatives, and at groups of people in specific program. She/he also collaborates in research projects.

SPECIFIC FUNCTIONS* Support and participate in the development and implementation of diabetes education within a multidisciplinary care setting;* Collaborate in the application and implementation of best practices for the care with users living with diabetes (Diabetes Canada Clinical Practice Guidelines, INESSS protocols, and guidance from other relevant organizations such as regional public health authorities);* Provide clinical support to health care providers involved in the care of users living with diabetes;* Support the development of health care providers' expertise in the field of diabetes through educational sessions and preceptorship;* In collaboration with partners, develop educational tools for clients with prediabetes or diabetes that respond to their identified needs and promote self-management;* Collaborate in the development and implementation of clinical documents to support the practice and used by health care providers;* Perform all other tasks related to his/her work as Diabetes Educator as required by his/her supervisor.

Education:

  • Bachelor of Science in Nursing or an undergraduate degree consisting of three (3) admissible certificates, of which two (2) are accredited certificates in Nursing;
  • Member in good standing of the O.I.I.Q.;
  • Certified Diabetes Educator (CDE) recognized by the Canadian Diabetes Educator Certification Board (CDECB) with certification obtained or renewed within the past five years is an asset.
    o Candidate meeting CDECB eligibility criteria and planning to attempt certification at the next examination session in May 2026 will also be considered;

Experience:

  • A minimum of 2 years or 800 hours of diabetes education practice within the past 3 years. This experience includes providing education, management advice, or care to individuals at risk for diabetes, those with prediabetes or diabetes, as well as their families or health care professionals.

KNOWLEDGE AND SKILLS* Knowledge and understanding of diabetes and its management, covering theoretical, pharmacological, and non-pharmacological aspects of care;* Familiar with the fundamental concepts, principles, and core values underlying motivational interviewing techniques and education science;* Competency in delivering educational activities to individuals and groups, with strong interpersonal communication, leadership, and teamwork skills;* Autonomous, likes to take initiative, flexible and empathetic;* Excellent critical thinking and decision-making skills;* Ability to effectively use technological tools such as virtual meeting platforms, educational software, and electronic health records to enhance learning and client care;* Knowledge of Cree culture and language, and social/health issues in Eeyou Istchee is an asset.

LANGUAGE* Fluent in English;* Fluency in Cree or French is an asset.

OTHER* Available to travel inside and outside of the Eeyou Istchee territory as required;* This position is teleworking;* This position is currently being offered telework, subject to operational needs. Please note that this is not a permanent condition of the position and the employer reserves the right to request regular physical presence in the office.
