SUMMARY OF THE POSITION

Person who designs, implements, updates, analyses and assesses activities in one or more social programs of the MSDC/CMC. The employee reports to the Coordinator in charge of MSDC program operations, is part of the local interdisciplinary teams of the MSDC/CMC and works in close partnership with the regional interdisciplinary teams of the Allied Health Professionals and regional HROs. The incumbent may play the role of team leader if required by the supervisor. He/she is expected to facilitate synergy in various social programs under the local MSDC and CMC mandates in order to avoid them working in overlap or silos modes.

SPECIFIC FUNCTIONS

• Carries out psychosocial assessment, care, counselling, assists patients and their family in setting personal goals by providing them with guidance and refers clients to the various resources and pathways available to meet their needs, including to appropriate professionals using the interdisciplinary approach and his/her HRO mandate;

• Conducts activities that includes the conception, updating, analysis and evaluation of one or more MSDC programs, including Kapatakaan, in partnership with the local CMC teams;

• Supports the development of MSDC protocols and guidelines and makes sure that these are followed, respected and adapted continuously to the clients’ needs ensuring MSDC and CMC mandates;

• In collaboration with the multidisciplinary team facilitates group discussion cases with the MSDC and CMC staff, plans to address the needs of various but separate categories of users, avoiding overlaps or working in silos with the local CMC teams;

• Identifies and answers MSDC team’s and clients’ needs;

• Works in close collaboration with the local management team to help improve the services, provides guidance and support to the clinic’s personnel when required by the supervisor;

• Creates and uses the material necessary to the application of the intervention or rehabilitation plans, including under Kapatakaan.

• Establishes a relationship of respect and trust with patients, their family and the community members and applies principles of communication and therapeutic alliance;

• Ensures the involvement and awareness of the family members and the surrounding community in the application of the intervention, rehabilitation plans or community-based programs or activities;

• Accompanies the clients all along their path to resolve psychosocial and emotional problems and allow the clienteles to increase their self-reliance, quality of life and social integration;

• Organises and manages packages of support to enable service beneficiaries to lead the fullest lives possible, informs about various coping strategies in regard to the issues in their personal and family everyday problems;

• Initiates, supports and carries out development projects necessary to meet the users’ needs in collaboration with local and regional partners within the organization;

• Provides statistics, indicators, and units of measures, and produces various types of reports;

• Carries out on call duties and all other related tasks upon the request of her/his immediate supervisor.

REQUIREMENTS

Education:

• Depending on the given sphere of activities, must have the required university degree in social sciences, such as criminology, social work, sexology or psychology;

• A human relations officer who has the required university degree in social work and is a member of the Ordre professionnel des travailleurs sociaux du Québec may use the title of professional social worker;

Experience:

• Three (3) years of relevant experience;

• Housing is not available.

Languages :

• Fluent in English and Cree;

• Fluency in is an asset.
