SUMMARY OF THE POSITION
Person who participates in identifying the needs of the population and uses the appropriate techniques to implement the various community action programs. She/he acts as a resource person for community groups.

SPECIFIC FUNCTIONS
The Community Worker works within a multidisciplinary team with a particular focus on promoting the social well-being of individuals, families and communities. The Community worker is involved in short-term follow-up for clients for situations that do not require specialized services, acting as a liaison to ensure the continuity of care for people while they are accessing Wiichihiituwin services. In collaboration with other professionals and service providers, they use a strength based and trauma informed approach to assess the needs of community members. They facilitate access to services that support increased social and psychological functioning and maximize their client's well-being. Tasks include, but limited to:

  • Uses a client centred, strength based, trauma-informed approach in all interactions with clients;
  • Provides short-term and solutions-focused interventions and assistance with situations that can be resolved in a few sessions;
  • Remains available to address the spontaneous needs of clients while under Wiichihiituwin services;
  • Engages in and contributes to case discussions with other professionals within the care team;
  • Responds to crisis situations with people in distress as needed;
  • Refers clients to appropriate service in the cases where long-term psychosocial follow-up or specialized services are required;
  • Maintains accurate documentation of the information collected using Care4 (progress notes, reports, forms, etc.);
  • Establishes connections with the CMC psychosocial team, team leaders, health care providers, liaison nurses, community support services;
  • Reports relevant information to the team leader and coordinator in timely fashion;
  • Ensures ethical and professional practice that prioritizes the well-being of clients (confidentiality, policies, clinical process, professional communication etc.);
  • Participates in an ongoing process of self-awareness, learning and growth to enhance one's professional competencies;
  • Attends required trainings and remains up to date on current standards of practice;
  • Performs all other related tasks as required.

REQUIREMENTS
Education:

  • Must have a diploma of college studies in social work techniques or social service techniques or another appropriate specialization from a school recognized by the Ministère de l'Éducation, du Loisir et du Sport;
  • This job title also includes those who have a certificate of college studies in social work techniques or social service techniques;
  • Applicants with an attestation of college studies in Educators in Native Childcare Services or Special Needs Education aboriginal and Inuit children may be considered;
  • Completed the Charlie Training or being willing to follow the training.

Knowledge and Abilities:

  • Knowledge of Cree culture and social/health issues in Eeyou Istchee;
  • Knowledge of Eenou/Eeyou Pimaatsiiun healing practices and paradigms is an asset;
  • Ability to communicate with people, to run interviews and to establish and maintain a "helping relationship with beneficiaries";
  • Ready to learn about 2nd line services ("Youth Protection" and "Young Offender Procedures");
  • Skills in counseling and Knowledge of the laws: Y.O.A, Y.P.A, L.H.S.S.S
  • Ability to work in collaboration with CLSC Community worker, NNADAP worker and other medical social services staff and other organisms within and outside the community;
  • Initiative, dynamism and sense of responsibility;
  • Must show initiative, be dynamic, exhibit responsibility and professionalism;
  • Must be sensitive to issues within the family unit.

LANGUAGE* Fluent in English;* Fluency in Cree and/or French is an asset.

OTHER* All JBNQA Cree beneficiary hired to work for the Wiichihiituwin direction either at the Montreal or the Val d'Or or the Chibougamau point of service are entitled to the following benefits:

o Three (3) or four (4) return flights per year for the employee and their dependants to go back to their affiliate community via CBHSSJB's charters, according to the collective agreement.
