Description
The pharmacist is responsible for providing pharmaceutical care and services to CHSLD Wales Inc. residents. They collaborate with various teams to provide quality care and services that respect a milieu de vie approach and residents’ rights. They maintain a partnership with the community pharmacy and contribute to the continuous quality improvement of pharmaceutical care and services.
**This is a temporary position until March 2027, with the possibility of renewal and permanent employment.
Responsabilities :
- Carry out the comparative medication review (BCM) in collaboration with the healthcare team
- Meet with residents and their loved ones to discuss pharmacotherapy
- Evaluate pharmacotherapy
- Develop the pharmaceutical care plan and agree on priorities in collaboration with the care team, the resident, and their loved ones
- Implement the pharmaceutical care plan and adjust as needed
- Provide quality comfort care in collaboration with the care team
- Optimize end-of-life medications
- Assume roles and responsibilities in a request for medical assistance in dying
- Participate in the collection and analysis of pharmacological and clinical process indicators
