The pharmacist is responsible for providing pharmaceutical care and services to CHSLD Wales Inc. residents. They collaborate with various teams to provide quality care and services that respect a milieu de vie approach and residents’ rights. They maintain a partnership with the community pharmacy and contribute to the continuous quality improvement of pharmaceutical care and services.

**This is a temporary position until March 2027, with the possibility of renewal and permanent employment.

Responsabilities :
  • Carry out the comparative medication review (BCM) in collaboration with the healthcare team
  • Meet with residents and their loved ones to discuss pharmacotherapy
  • Evaluate pharmacotherapy
  • Develop the pharmaceutical care plan and agree on priorities in collaboration with the care team, the resident, and their loved ones
  • Implement the pharmaceutical care plan and adjust as needed
  • Provide quality comfort care in collaboration with the care team
  • Optimize end-of-life medications
  • Assume roles and responsibilities in a request for medical assistance in dying
  • Participate in the collection and analysis of pharmacological and clinical process indicators

Requested skills

  • University degree in pharmacy or pharmacotherapy
  • Active member of the Ordre des pharmaciens du Québec (OPQ)
  • Two years of CHSLD experience are an asset
