The Complex Care and Recovery Program is currently seeking a full-time, temporary (1 year) Team Lead for the Psychosis Recovery Inpatient Treatment Unit 6 (PRT 6). Reporting to the Manager, and working collaboratively with the Complex Care and Recovery practice leadership group, the Team Lead supports the Manager to anticipate needs of the outpatient clinic over time and collaborates with the clinic’s interprofessional team to provide effective coordination of client/patient-centered care, consistent with CAMH’s vision and strategic directions. The Team Lead will assume responsibility for Intake Coordination, communication, liaison with internal and external stakeholders, scheduling of intake assessments and client/patient admissions. As part of the Team Lead’s responsibility, the successful candidate will provide case management, liaison and coordination of client/patient discharges, transfers, and client assignments. In support of programming, the Team Lead will co-facilitate groups. The successful candidate will facilitate urgent service delivery thru Rapid Access and ensuring a high standard of quality and safety. The Team Lead will ensure that the client/patient assignment is clear and reflects the principles of accountability for care, also ensuring that client/patient assignments are accurate and properly recorded as per established clinic’s processes and CAMH standards. The Team Lead will coordinate with the manager regarding staff schedules to ensure appropriate coverage of the clinic. In collaboration with practice leads you will link new staff, with appropriate clinic resources and facilitate orientation to the clinic, ensuring they become familiar with the clinic’s procedures/processes. You will coordinate and/or assign, in collaboration with clinical supervisors/preceptors, an appropriate CAMH staff to assist students of all disciplines in providing a safe and respectful learning environment. The Team Lead will collaborate with the manager regarding staff participation in off-unit meetings/ trainings, educational events. You will ensure that the monthly admission/discharge reports (Patient Flow Data) are accurate and follow appropriate clinic procedures, transfer of care report and other relevant reports, as required. You will support a healthy workplace that embraces diversity, promotes positive team dynamics and reinforces collaborative practice, complies with all applicable standards, requirements and contribute to the development, implementation and evaluation of best-practices, all of which advance excellence in nursing and client/patient care. This position will be based at the Outpatient clinic site and will be a regular day shift.
The successful candidate must be registered in good standing with the College of Nurses of Ontario and possess a current B.C.L.S. (CPR) certificate. Preference will be given to candidates who possess or are currently pursuing a BScN and CNA certification in Psychiatric/Mental Health Nursing. At least two years (or 3900 hours) of clinical experience within the mental health and additions field is preferred; with a particular focus on clients with psychosis and specific knowledge of the recovery-based model of care. The successful candidate will possess proficiency and leadership in problem-solving, decision-making, conflict resolution, negotiation, communication, delegation, critical thinking, team building and interpersonal skills. The successful candidate will possess the ability to think at a program-level and participate in unit-level as well as organization-wide change initiatives. The individual will have the ability to communicate effectively with the team, including management, both verbally and in writing; knowledge of the roles of interdisciplinary team members, their professional responsibilities and areas of expertise is essential. Demonstrated expertise in maintaining a culture of safety for staff, clients/patient, families and visitors according to CAMH and regulatory standards is critical. Bilingualism (French/English) or proficiency in a second language would be an asset.
