This individual will collaborate with internal teams, external partners, and Ontario Health Teams (OHT) to foster cohesive, patient-centered care across the health system. The ideal candidate will have expertise in system integration, collaborative healthcare delivery, and leading innovative interprofessional primary care models that address the diverse needs of patients and communities.

Qualifications:

o Post secondary education in Healthcare required.
o Regulated Health Professional with nursing or other allied health background an asset.
o University Degree in Healthcare, Business Administration an asset.
o Masters Degree in Health, Business Administration an asset.

Skill, Ability and Experience:

o Minimum of 5 years of experience in healthcare management or program leadership, including direct experience in integrated care models, collaborative health systems, or cross-functional healthcare teams.
o Nursing or other health care experience, working in interprofessional team-based care setting.
o Experience with Ontario Health Teams or similar collaborative models is an asset.
o Strong leadership and interpersonal skills with the ability to influence and motivate teams.
o Deep understanding of healthcare systems, interprofessional care team models, and patient-centered care.
o Proven ability to manage complex projects, budgets, and stakeholder relationships.
o Excellent communication skills (both written and verbal) with the ability to engage diverse audiences, including senior leaders, staff, and community partners.
o Strong analytical skills with experience using data to inform decision-making, improve performance, and guide strategic planning.
o Knowledge of healthcare policies, regulatory standards, and funding frameworks as they pertain to integrated care and collaborative health models.
o A collaborative mindset with a passion for fostering teamwork across diverse disciplines and organizations.
o Strategic thinker with the ability to navigate complex challenges and drive innovative solutions.
o Ability to work independently and manage multiple priorities in a fast-paced environment.
o Strong problem-solving skills with a focus on continuous improvement and achieving measurable outcomes.
o Willingness to travel within the region as required.
o Flexibility to work outside of regular hours, as necessary, to meet project timelines and organizational needs.

Principal Duties:

• Leadership in Integrated Interprofessional Primary Care Models: Lead the design and implementation of integrated interprofessional care team models and programs that address the needs of diverse patient populations, including those requiring primary care, mental health, and other community-based services. Provide oversight and leadership to the Preventive Care Programs within the Sarnia Lambton OHT.
• Collaborative Health Systems: Build and sustain strong partnerships with healthcare providers, mental health professionals, addiction specialists, and community organizations to create seamless patient-centred primary care referral pathways and improve health outcomes.
• Project Oversight & Coordination: Oversee the execution of key projects and proposals aimed at implementing and scaling interprofessional primary care models. Ensure alignment with organizational goals, regulatory standards, and funding requirements.
• Stakeholder Engagement: Engage with a broad range of stakeholders-including healthcare teams, government bodies, primary care teams and community organizations-to align efforts, share resources, and promote collaborative interprofessional care practices.
• Innovation & Best Practices: Lead efforts to identify innovative approaches to integrated interprofessional care and collaborative health delivery. Support the adoption of best practices, evidence-based models, and continuous improvement strategies within the team and across the system.
• Strategic Management: Drive the strategic development and coordination of healthcare services within the context of integrated interprofessional primary care models, ensuring alignment with the goals of Ontario Health Teams and Ontario Health regional healthcare priorities.
• Performance Monitoring & Evaluation: Develop and implement systems to monitor and evaluate the effectiveness of interprofessional primary care models. Use data and feedback to inform improvements and measure outcomes related to patient care, team collaboration, and overall service delivery.
• Training & Capacity Building: Provide training, support, and mentorship to staff and partners to build capacity for integrated interprofessional care within organizations. Foster a culture of collaboration, learning, and accountability.
• Advocacy & Policy Support: Advocate for integrated care solutions within policy discussions and provide insights on how to improve collaboration across the healthcare system. Assist with policy development or adjustments to better support integrated models of care.
