Description
Assisted Living Alberta is dedicated to Enhancing Care in the Community, helping people be healthy, well and independent in their homes and communities. For more information Assisted Living Alberta. Edmonton Zone Continuing Care provides services through five main program areas: Home Living, Supportive Living, Facility Living, Transition Services and Palliative Care. We are looking for a Speech Pathologist to join our Facility & Supportive Living team to provide care and support to adults and seniors residing in congregate living settings. Supportive Living is a non-institutional alternative for those who are no longer able to live in their own home, which focuses on health promotion, well-being, independence and maintaining functional abilities. This position works within a collaborative multidisciplinary team consisting of various healthcare professionals from the nursing and allied health professions. This team collaborates and conducts home or site visits to deliver client-centered services to individuals with a wide variety of physical, cognitive and mental health conditions, including complex health needs. Using strong clinical and interpersonal skills, you will implement speech therapy services in accordance with regulatory and ALA practice standards, using best practice and a client-centered approach, engaging the client, family/caregivers, physicians, broader health care team and other internal program or external agencies as indicated. Speech therapy services may encompass any combination of direct therapeutic services, appropriate education, and/or connection to needed resources. Through the development and/or contribution to the Care Plan, in collaboration with the client and other healthcare providers, you will ensure services are provided and modified, based on the client’s response to the intervention.
As a Speech Language Pathologist, you will provide clinical professional practice both independently and as a part of a collaborative team, which may include external partners. You will be responsible for providing prevention, health promotion, identification, assessment, intervention, and case management. You will participate in the planning, coordination and delivery of rehabilitation services in collaboration with patients/clients, families, healthcare professionals and other partners. Your leadership and interpersonal skills will aid you in supervising students and providing clinical guidance to other staff as appropriate. You will use your clinical expertise to inform ongoing quality improvement, service integration, and program development.
Master's degree in Speech-Language Pathology. Active or eligible registration and practice permit with the Alberta College of Speech Language Pathologists and Audiologists (ACSLPA).
Basic Cardiac Life Support- Healthcare Provider (BCLS-HCP). Experience with program development, implementation of educational programs and change management. The successful candidate will work independently in clients’ homes, which could include congregate settings, single family dwellings, condos and/or walk-up apartments. Therefore, the successful candidate must be able to work in different settings and environments (home environments and weather conditions), as well as be able to lift and carry medical bags, which weigh approximately 25-30 lbs. At the time of interview, you may be required to demonstrate your computer knowledge and proficiency by completing a hands-on assessment. You must demonstrate sufficient typing speed to meet workload demand and navigate through various computer programs including Microsoft Office (Word, Excel, Access, PowerPoint), Visio, Outlook and Internet.
Membership in Speech-Audiology Canada. Training and experience in the area of dysphagia management. McMaster Case Management training and understanding of the ALA Continuing Care Case Management Competency Framework. Knowledge of the principles of adult learning. Connect Care Experience. Previous supervision of students.
