Description
The Sheldon Chumir Health Center (SMC HC), located in downtown Calgary, offers a wide range of services including urgent care, mental health, public health, home care, specialty clinics and community outreach. The Urgent Care Center (UCC) is staffed 24 hours a day, seven days a week and provides care for unscheduled health conditions to a diverse population, as part of an integrated community service delivery model. Reporting to the Manager of the Urgent Care, the Social Worker (SW) III works primarily in the Urgent Care Centre. This position offers the opportunity to be flexible, adaptive, and innovative in providing a client and family-centred approach to patient care. The Social Worker III will provide assessment, treatment, consultation, education and/or case management based on client need. The SW identifies the need for and facilitates community supports and service partnerships to address complex, co-morbid client and/or family needs. This is position with expectations that the incumbent will play a role in program development and quality improvement. The incumbent will work closely with all members of the Urgent Care staff and physicians as well as with the Urgent Mental Health Team, and with other providers in the community and AHS.
As a Social Worker III, you will perform either advanced level assessment and treatment or act as a clinical lead. You will work with or within teams to provide health care services for patients with complex and multidimensional medical, psychosocial, environmental and instrumental needs. You will provide advanced clinical services, consultation to the interprofessional team and patients/families and supervision of students and team members as assigned. You may also act as a clinical lead providing leadership in the planning, development, and delivery of services in a specific area, as well as staff development, quality improvement and evaluation or research activities as identified by the needs of the service area(s).
Master's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
Recent experience in medical social work, with emphasis on crisis intervention required. Previous experience and training in crisis work required. Ability to work in a high stress, dynamic environment. Recent experience in working with patients/families facing chronic and/or life-threatening illness required. Demonstrated ability to work independently and be creative. Experience and advanced skill in providing therapy to individuals and families. Demonstrated ability to function effectively with interdisciplinary team required. Demonstrated interest in research and evidence-based practice. Demonstrated strong interpersonal and communication skills. Flexibility required.
