Assisted Living Alberta at the Sturgeon Community Hospital is looking for a Social Worker to support patients and families in inpatient/emergency department settings and be an integral part of department changes. This position requires a high degree of critical thinking, strong knowledge of Social Work scope and role, and a passion for social work excellence. You will be required to prioritize workload, formulate assessments and create intervention plans in a short timeframe. These plans include psychosocial and financial interventions, risk management, and resourcing to support ongoing treatment and/or discharge planning. The Social worker will demonstrate exceptional communication skills and ability to work collaboratively with caregivers, families, and community organizations. This position works collaboratively with multidisciplinary teams and requires competence in approaching questions about decision-making and capacity. This position is highly involved with processes, services and resources related to alternate levels of care.

As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).

Minimum 2 years of experience working in Acute Care as a Social Worker. Relevant clinical experience may be considered. Experience supporting healthcare teams in the areas of mental health, capacity and decision-making. Experience working and supporting patients and families to manage risk. Experience navigating ethics and ethical decision-making. Experience with supportive counseling. Experience collaborating with hospital transitions and community housing providers.
