Description
Reporting to the Urgent Care Manager, the Social Worker provides a broad range of Social Work services to community members, outpatients and their families accessing the Airdrie Urgent Care Centre and Airdrie Community Health Centre. The service is primarily supportive in nature and requires experience in psychosocial assessments, counselling, and resource navigation. This position also involves consultation and collaboration with Airdrie Urgent Care Centre staff, community physicians and agencies, and requires the ability to work independently with occasional review, consultation and evaluation from a manager. The incumbent must be able to work as part of an inter-disciplinary team and commit to strengthening team dynamics by actively facilitating inter-professional collaboration to enhance patient health outcomes.
As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.
Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
Experience in a health care setting required. Short-term, one-time session counselling experience required. Recent experience with individuals presenting with psychosocial issues and working with acute and chronic mental health populations required. Skills in the delivery of grief, single session and/or short term therapy. Demonstrated extensive knowledge of community resources required. Demonstrated ability to function effectively within the inter-disciplinary team. Strong assessment and counselling skills required. Ability to establish positive working relationships with a wide variety of clients and families. Ability to organize and set priorities. Excellent communication skills, written and verbal. Considerable initiative and judgment required.
Experience in an ER/UCC setting preferred. 3-5 years recent social work experience preferred. Recent experience in working with patients/families facing traumatic situations preferred.
