Our mandate is to support individuals in maintaining independence, dignity, and quality of life through integrated care solutions that meet their unique needs across home, facility, and hospital settings. We are seeking a Social Worker II to join our Central Zone Transition Social Work Team in the Stettler area. This role provides professional social work services in clients’ homes, continuing care facilities, and hospital settings. As part of a multidisciplinary team, you will: Conduct psychosocial assessments, identify unmet needs and develop care plans, provide social navigation and resource-based interventions, collaborate with clients, families, and decision-makers to achieve client goals. Key Responsibilities: Crisis intervention, education, advocacy, and resource navigation financial assessment for continuing care placement, risk screening, counselling, transition and discharge planning, supporting care partners in navigating health care resources, relevant education and cognitive screening.

As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).

Strong communication and collaboration skills, ability to work independently in diverse settings (homes, congregate living, facilities, hospital) professional judgment, initiative, and time management, proficiency in Microsoft Office Knowledge of seniors’ benefit programs and relevant legislation: Guardianship and Trusteeship Act, Personal Directive Act, Enduring Power of Attorney Act.

Experience in Continuing Care settings. Familiarity with Connect Care. Palliative Care experience. Opportunities for professional growth and development.
