As a Social Worker II, you will be responsible for a wide range of social work duties including conducting psycho-social assessments, formulating collaborative treatment plans, delivering restricted psychosocial interventions, providing holistic case management, consulting with care team members and community partners to meet the needs of clients experiencing chronic mental health needs within Addiction & Mental Health programs. The Social Worker II facilitates appropriate referrals to internal and external parties to meet clients and their families psychosocial and instrumental needs, such as housing, food, transportation, income support, and financial management and liaising with community agencies/programs. The Social Worker II will also a key in establishing connections with community agencies to engage vulnerable clients collaboratively. This position has transitioned to Recovery Alberta effective September 1, 2024. As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).

2 years' experience working within an outreach capacity. 2 years' experience working with community social service agencies. Demonstrated experience working with complex addiction and mental health related issues.
