Description
Across Cancer Care Alberta (CCA), Supportive Care Services are delivered by a multidisciplinary team - including social workers - who support individuals and families from diagnosis through treatment and beyond. The Social Work team at the new Arthur Child Comprehensive Cancer Centre (ACCC) needs a permanent full-time Social Worker II (SWII) to join the team. Social work plays a vital role in helping patients and caregivers navigate the social determinants of health. Under general supervision of the Manager, the Social Worker’s primary responsibilities include: Providing emotional and practical support to individuals and families. Developing and implementing care plans tailored to clients' needs. Collaborating with other professionals and community resources. Advocating for clients' rights and access to necessary services Triaging and screening referrals, providing bridging support (supportive counselling) Co-developing and facilitating psychoeducational groups Orienting and educating new staff and trainees Participating in local and provincial quality improvement initiative and service evaluation. Engaging in continuous professional development, mentoring others and strives to work to their full scope of practice. Providing cross-coverage across the inpatient and outpatient Social Work teams to ensure timely access to care and reduce barriers to cancer treatment. Hours and caseloads associated with Supportive Care positions may change based on operational needs. This position will start as part of the outpatient team and will eventually orient to help cover inpatient units.
As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.
Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
Basic Cardiac Life Support-Healthcare Provider (BCLS-HCP) through Heart & Stroke Foundation. A minimum of 2 years post-degree (Social Work) experience. Within the last year, experience working in an urban adult cancer care setting. Strong knowledge of the social determinants of health and their impact on cancer care. Experience with psychosocial assessments, supportive counseling, and crisis intervention. In-depth knowledge and experience of community resources, financial supports, and system navigation. Experience providing grief and loss support. Demonstrated skills in assessment, intervention, system navigation and case management. Demonstrated dedication to professional development. Proven ability to work in interdisciplinary teams and collaborate across care settings. Commitment to equity, diversity, and inclusion in patient care. Proficient with EPIC/ Connect Care. Proficiency in Microsoft Office applications, including PowerPoint, Outlook, Word, SharePoint, and Teams.
Five years’ experience working within a chronic disease management framework with adult populations in an urban acute care setting. Experience facilitating groups and providing psychoeducation. Experience with and knowledge of cancer-specific community and financial resources. Experience developing relationships with community partners. Within the last two years, experience provided virtual care. Training or certification in trauma-informed care, suicide risk management and/or palliative care.
