Description
An exciting opportunity exists with Integrated Home Care - Consult and Treat Team. You will get to work with clients throughout the Calgary zone. As a Social Worker II (SWK II) you report to the Area Manager and the Care Manager of the Consult and Treat Team in the Integrated Home Care program and provide a range of social work services to clients, families and an interdisciplinary team. The IHC program supports clients to remain independent in the home. You will be required to operate a laptop computer and mobile phone, and for manual handling (lifting, lowering, carrying, pushing and pulling) as a regular part of the job. For example: lifting of approximately 30 pounds in and out of a vehicle on a regular basis and carrying this weight up and down stairs throughout the day is common within this job. Learning resources and training are provided to safely perform work.
As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.
Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
Minimum 2 years social work experience in within a community setting within the last 5 years. Considerable initiative and judgement and the ability to work independently in community settings is essential. Basic computer skills.
Excellent communication skills, written and verbal. Experience working within an interdisciplinary team environment is preferred. Knowledge of and experience with guardianship, trusteeship and capacity assessment processes is an asset. In-depth knowledge and experience in working with Income Security programs at community/provincial/federal levels (Alberta Works, AISH, CPP) is preferred.
